COVID19 - service available * drivers are available to provide FREE rides to COVID-19 vaccine appointments * arrange a ride with us to your appointment by calling 1-888-255-6680 or by completing the Request Service form on the website

Provides clients with rides to and from essential destinations
  • medical appointments
  • shopping, errands, and appointments
  • to access CCD programs
  • drivers can be CCD staff or volunteer drivers
Access to Primary Care
  • provides a Personal Support Worker for clients who need to attend medical appointments and have mobility or other related challenges
Accessible Vans
  • wheelchair accessible vans driven by CCD staff for clients with mobility issues
  • staff push client up ramp, secure wheelchair and bring down ramp
  • client has to be able to wheel themselves to ramp or have a caregiver do so
  • must travel with escort if cannot travel alone
Home at Last
  • provides Personal Support Worker to bring client safely home from hospital if aging or has special needs and would benefit from assistance in getting settled
Eligibility
Those unable to use public transportation because of needs related to aging, physical and/or mental health and other options are not available
