- Offering support and strategies to better manage chronic illnesses like diabetes, depression and asthma
- Working with patients and their families to navigate the healthcare system and coordinate better care
- Eligibility
- Individuals must belong to Dufferin Area Family Health Team to access services
- Must be referred by a family physician or go through Health Care Connect 1-800-445-1822 for a new doctor
