- Provides a long-term case management program that assists individuals and families to find and maintain housing
- Works to address and mitigate barriers to achieving permanent housing
- Offers support, advocacy, and accompaniment to appointments
- Initiates referrals to other programs and services
- Provides ongoing outreach after permanent housing has been established
- Comments
- Outreach workers provide weekly home visits to ensure the individuals are supported in their homes, and to help address any housing or tenancy issues.
- Intake is completed by the Homelessness Network and then a case manager is assigned to the client.
