Description
  • Provides a long-term case management program that assists individuals and families to find and maintain housing
  • Works to address and mitigate barriers to achieving permanent housing
  • Offers support, advocacy, and accompaniment to appointments
  • Initiates referrals to other programs and services
  • Provides ongoing outreach after permanent housing has been established

Comments
  • Outreach workers provide weekly home visits to ensure the individuals are supported in their homes, and to help address any housing or tenancy issues.
  • Intake is completed by the Homelessness Network and then a case manager is assigned to the client.
