Description
Ongoing telephone support and Caller Reassurance Program for isolated seniors, including safety check-ins, medication reminders, social calls, emotional support, companionship, referrals to community services

Works with clients to reduce feelings of isolation and loneliness, improve positive coping skills
Eligibility
Adults 55 years and older, particularly isolated and vulnerable seniors

Residency Requirements: Central West Local Health Integration Network (LHIN) catchment area
