- Description
- Offer nurses, personal support workers and other support professionals to make up the difference between what is provided and what is needed.
- Visiting Nursing - assess individual needs and provide a plan of care; provide service in wound care, diabetes management, medication management, health education and more.
- Shift Nursing - provide nurses in 4-12 hour shifts to enable individuals to be cared for in their own home
- Personal Support Services - assistance with personal care and the support required to remain in the home
- Respite Care - provide caregivers with a break from their regular care giving duties
- Foot Care Services - assessment, care and advice for those having difficulty taking care of their feet
- Ocupational Therapy - offer home safety assessments and prescriptions for mobility aids and applications for the Assistive Devices Program
