- Description
- Non-emergency, community-based service supports vulnerable patients living independently in the community. Aims to reduce 911 calls, emergency department visits and hospital admissions. Services include:
- in-home assessments
- safe home assessments
- scheduled wellness checks
- remote patient monitoring
- direct communication with primary care providers
- diagnostic tests
- Eligibility
- Patients who require additional assistance while living in their home, including high-risk clients living with complex medical issues such as diabetes, COPD and CHF
- Application Process

Community Paramedics will make initial contact with the patient within 24 hours after receiving a referral to discuss
