Services that support adults and their caregivers with needs related to aging, physical and/or mental health

Exercise and Falls Prevention
  • offered at various locations in the community https://www.communitycaredurham.on.ca
  • maintain and enhance aerobic fitness, increase functional fitness, improve strength and muscular endurance, work on flexibility and maintaining joint range of motion, develop better balance and stability
  • enhance overall physical and mental health
  • socialize and build connections with others in their community
Footcare Clinic
  • provided by nurses trained in foot care
  • 15 minute session
  • cutting nails
  • paring down calluses and heels
  • removal of corns
  • a foot massage with lotion
  • basic foot care monitoring
Friendly Visiting
  • volunteers will call clients on a regular basis who are isolated to share in some pleasant conversation
Luncheon Out
  • opportunity for adults with physical disabilities and the frail elderly to enjoy a hot nutritious meal
  • entertainment and health promotion/wellness demonstrations
  • luncheons are held at preset times
  • transportation can be arranged
Telephone Reassurance
  • telephone reassurance calls may last between 5-7 minutes for a friendly check-in and brief chat
  • security calls are intended for check-ins on well-being and follow a different protocol than social calls
Adult Day Program
  • daily functional fitness
  • use of Technology including iPads and Smartboards
  • wellness programs
  • reminiscing/discussion Groups
  • arts and crafts
  • educational and interactive games
  • outdoor programs
  • music therapy
  • entertainment/special themed events
  • multi-age programs
  • pet therapy
