- Eligibility
- Seniors, 65 years of age or older, who require assessment and interventions from at least two of the following disciplines: occupational therapy, nursing, physiotherapy, social work, etc.
The program is suitable for patients who are independently mobile with or without a mobility aide (cane, walker, wheelchair) and those who require minimal physical assistance. Persons who require constant supervision or may demonstrate inappropriate social behaviour cannot be managed in the program.

- Referral from family doctors, members of the Geriatric Assessment Outreach Team, or referral through an in-patient admission to the Queensway Carleton Hospital * Please bring a small, recent photo of yourself for our records.
