- Contact the Department of Biomedical and Molecular Sciences at the time of death * a full medical history will be obtained and if the donor meets the acceptance criteria, arrangements for transportation can be made through a funeral home or a cremation service The Anatomy Act Form found on the website may be completed by the donor in advance and forwarded to the Human Body Donor Program at Queen's University
