- Transportation to medical appointments or errands
- Eligibility
- Indigenous individuals and families living within the jurisdiction of Dilico and who meet the criteria as determined by Care Managers
- Application Process
- Verbal or written referrals accepted from Dilico employees or First Nations communities receiving Community and Personal Support Services
- Requires an assessment with a Home and Community Care Manager
