- Offers health check-ups with a Nurse Practitioner twice per month
- Provides advice on health issues and offers referrals to other health providers and services

- Patricia Gardens residents and community members 60 years of age and over
Must meet one of the following criteria:- The client is in receipt of or eligible to receive other long term care community services
- The client is isolated and requires social interaction or stimulation
- The client is in receipt of or eligible to receive other long term care community services
