- Provides education and services for the prevention, management and treatment of diabetes
- Provides case management and screening services
- Provides basic foot care services
- Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
- Advises on developing and implementing an approach to care and management of diabetes including monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices
- Assists individuals in finding appropriate services
- Provides referrals to other health care professionals as needed

- Members of Zhiibaahaasing First Nation
- Adults 18 years and older with pre-diabetes, type 1 diabetes, and type 2 diabetes
- Members of Zhiibaahaasing First Nation
