- Description
- Provides education and services for the prevention, management and treatment of diabetes
- Provides case management and screening services
- Provides one-on-one or group consultation on monitoring blood sugar levels and medications
- Offers basic foot care
- Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
- Advises on developing and implementing an approach to care and management of diabetes including healthy eating, exercise, and other lifestyle choices
- Assists individuals in finding appropriate services
- Provides referrals to other health care professionals
- Participates actively in the Integrated Diabetes Care Team with Nipissing and Temagami First Nations which offers quarterly clinics in each community to improve continuity of services that includes ongoing screening, diabetes management, education, medical supervision and referrals to specialized services
- Eligibility
- Members of Dokis First Nation 18 years of age and older living with pre-diabetes, type 1 diabetes, and type 2 diabetes or at-risk
