The Home and Community Care Support Services - SW provides School Health Support Services on a priority-needs basis. HCCSS professionals assess and prioritize students according to need. Children with complex needs who require assistance in order to safely attend school are the top priority.

Through the School Health Support Services program, a HCCSS Care Coordinator may arrange in-school access to a range of services.


For students in public schools, private schools or students who are home schooled, the HCCSS may provide:

* Nursing services * Dietetic services * Information and referral services * Coordination of services

For students in private schools or students who are home schooled, the HCCSS may also provide:

* Occupational therapy * Physiotherapy * Speech-language pathology * Personal support

Formerly the South West Community Care Access Centre (CCAC)

Health care coordination service * coordinates a wide range of in-home services to eligible clients * care coordination, nursing, physiotherapy, occupational therapy, social work, personal support * supportive care team for palliative and cancer care * elder abuse * nutritional counseling, speech language pathology and medical supplies and equipment * provides placement in long-term care facilities and respite care * facilitate application to residential hospice * assessments for government funded day away programs and supportive housing services * home and school health services for children with special needs * Care Coordinators determine eligibility for drug coverage under the Ontario Drug Benefit (ODB) Plan

Stonebridge Life Care & Wellness Services provide customized home and community care emphasizing the importance of maintaining and enhancing quality of life throughout the aging process.
