Helps maintain independence in home and community with various programs including:

SMART (Seniors Maintaining Active Roles Together) Exercise Program - health education and exercise for seniors and older adults

Visiting Programs - Volunteer visitors are matched one to one with seniors and adults with disabilities to provide companionship and support (Listowel); Palliative Care Volunteer Program(Perth East & Perth West)

Adult Day Programs - social, recreational, and therapeutic activities for older adults in a group setting for part of the day that helps to support family caregivers who provide day-to-day care

Meal Programs- Meals on Wheels includes Hot and frozen meal options; Congregate Dining programs(Listowel)

Transportation: Volunteer and Accessible transportation services for seniors and adults with disabilities in North Perth

COVID19 - Service Change - Program modified to Dining To Go with a meal delivered to your home and virtual social time. Additional nutritional support is available through Meals on Wheels.

Dining for Seniors is a community dining program intended to increase opportunities for nutritional and social support among seniors.

COVID19 - Service Change. ONE CARE is providing essential services. Continuing programs include Assisted Living, Meals on Wheels, Lifeline, Home Care, Home Help, Home at Last and In Home Respite. Modified programs (may be offered virtually, by phone or in a totally different way) include Transportation Friendly Visiting, Adult Day Program, Overnight Respite, Social Work, Dining (Dining to Go) and Exercise & Wellness (offered virtually). Programs on hold include Blood Pressure Clinics. In cases where a program is on hold or has had to be modified ensuring that client's needs are considered and working with clients and caregivers to determine how to support going forward.

Supports seniors and persons with disabilities to maintain their independence. Services include:

Hot meals delivered 5 days a week, at noon weekdays. Frozen meals available for pick up or delivery

Enjoy a delicious three-course meal from Mondays to Fridays at noon at the Adult Recreation Centre at 185 King St. S * accommodations will be made for special dietary needs

Home-style meals served 6 days a week

• Eat-in meals include an appetizer, main course, dessert and tea or coffee
• Reserved take-outs are available for regular meals
• If a diabetic dessert is required, please inform at time of booking

Aboriginal Health Access Centre providing integrated services for Indigenous people, families and communities * health professionals assess and treat non-life-threatening injuries or illnesses * health promotion, traditional healing, mental health and addiction services provided * dental services, Aboriginal midwifery, Indigenous palliative care * referrals to other local health services and personal development groups available on and off reserves * culturally safe, accessible services that combine traditional Indigenous and Western medical practices

Home-cooked healthy meal support for senior shut-ins, folks recovering from surgery, and bereavement support. Food program is designed to help alleviate the stress of shopping, prepping, and cooking when a person or family is struggling with other emergency issues.

Note: Intended for one-time offering of 5 home-cooked meals

Volunteers pick-up clients and escort them to lunch at a local restaurant with other clients and volunteers * limited seating available

2 outings per month * no outings scheduled in January, February, July and August

Packages of 7 frozen entrees available for delivery once per week

Clients choose from a menu * a variety of diet types are available * soups and/or desserts packages of 7 are also available
