Immediate crisis telephone counselling

Counsellors are skilled in Brief Solution Focused Therapy and Crisis Intervention to provide the following:

• Crisis Outreach and Support Team (COAST) referrals
• connection to addiction services
• safety and crisis planning
• coping skills and self-soothing management
• referrals and connections to on-going service as required
• Naloxone kits and harm reduction supplies

An assessment and treatment program for seniors living in the community who are experiencing mental health concerns which may include: depression, mood changes, dementia and/or anxiety. The service is focused on the individual and their family. The team provides a comprehensive geriatric assessment that will include a medical and mental health history and screening. The care plan is developed and implemented with the individual and their support network including their primary care physician.

Regional service to assist family, community and long-term care providers in the continuing care of older adults with complex cognitive, emotional and behavioural disorders

• specialized assessment, consultation, treatment and short-term specialty case management and intervention
  
• education and community/program development, community homes, retirement, and supportive housing and long-term care facilities

Wait times can be upwards of 5-6 months to see a psychiatrist

This regional service supports seniors who have complex medical and/or mental health needs. A team member follows up with the senior in their home once a comprehensive geriatric assessment has been completed and it has been agreed to by the senior and the care team that this service can support the individual's plan of care. The team member works closely with the senior's support network and promotes independence, self-directed care, and quality of life through the coordination of appropriate services and support for the senior.

Specialized psychiatric care and support for older adults who suffer from late onset mental illnesses or severe behavioural responses * program focuses on assessment, treatment, rehabilitation, prevention, family and community support and education *

Geriatric Day Program - Offers various professional programs and services including cognitive neuropsychiatry, mood disorders and psychotic disorders clinics, as well as limited therapeutic programs and therapy for outpatients

Discharge Liaison Team - Facilitates the transition of current inpatients or registered outpatients undergoing transition from hospital to the community

Regional Psychogeriatric Program - Provides clinical assessments in the patient's home, community clinic or long-term care facility, for older adults with behavioural or mental health issues

Psychiatry consultation, assessment and treatment services for older adults

All patients see a psychiatrist as part of the initial evaluation * some inpatient consultation and education to doctors, health care professionals and long-term care facility staff

Specialized care of older persons * evaluation and treating of functional, medical, and psycho-social aspects of illness and disability

Consultation, education, community development and evaluation of services * linked to older adults evaluation teams in ten counties by the Southwestern Ontario Geriatric Assessment Network

Geriatric Ambulatory Clinics - Consultative outpatient services and specialty clinics

Geriatric Rehabilitation Day Hospital - Hospital-based outpatient day program for frail older adults * offers nursing, physiotherapy, occupational therapy, and social work services to help patients be healthy, independent, and maintain their quality of life * program collaborates with community services to ensure the right supports are in place for frail seniors at home

Geriatric Rehabilitation Unit - 30-bed hospital inpatient rehabilitation program for older adults with multiple, complex health problems * goal is to promote patient's health, functional independence, and quality of life * services provided by doctors, nurse practitioner, nurses, physiotherapists, occupational therapists, physiotherapy/occupational therapy assistants, therapeutic recreation specialists, speech language pathologists, audiologists, dietitians, and social workers

Inpatient Unit - Inpatient clinical consultation, assessment, treatment, education, research and evaluation

Outreach - Peer support groups, education, and training for family members, service providers, and caregivers * sessions may include information such as advanced directives and safety issues

Physical Maintenance Program - Half-day program aimed at maintaining the client's physical function and mobility conducted by a rehabilitation trainer and a therapeutic recreation specialist with the assistance of volunteers

Regional Geriatric Program - Assessment of frail older adults in homes, hospitals, and long-term care homes * outreach team provides recommendations to care providers, families, and patients

Specialized behavioural support for children, adolescents, adults, and seniors who are experiencing behavioural, social, emotional, and psychiatric difficulties

Family Services - Provides one-on-one support for families * support focuses on addressing and stabilizing challenging behaviours * no waitlist * services provided wherever client is, including home, hospital, school or in any other community setting

Specialized Behavioural Support Services - Customized support provided in a variety of settings, including residential facilities, day programs, hospitals, schools, long term care facilities, foster homes, and community programs * flexible, client-centered services reviewed on a regular basis to ensure quality care * supports to youth justice programs for youth aged 12 to 17 who are in conflict with the law, to assist with community integration and support

Ambulatory Care Program (AC) provides specific short term out patient services that offers supportive care to the patient transitioning from acute in patient care to discharge or home environment * clinics provide services to the community with issues of dementia, delirium, memory, or falls

• Crisis Assessment and Support Team (CAST) - mental health crisis support telephone line staffed by experienced mental health professionals. Provides face-to-face assessments and short-term follow-up crisis counselling. Response time to clients is expected to be within 24 hours
  
  
• Mobile Crisis Rapid Response Team (MCRRT) - program partners a uniformed OPP officer with an experienced mental health professional to respond as first responders to calls through 911
  
  
• Specialized Geriatric Services - provides non-emergency clinical assessment, consultation, treatment and education to older adults, their families, and service providers who are, or know of, someone who is experiencing mental health issues combined with age-related difficulties
  
  
• Addiction Mobile Outreach Team - provides support to individuals living with substane use concerns, problem gambling or concurrent disorders through engagement, assessment and prevention through education
  
  
• Mobile Mental Health and Addictions Clinic

Community based mental promotion and education services for individuals and families facing mental illness and developmental challenges. Services include:

• outreach / case management
• specialized psychiatric and mental health assessments and treatment for adults and seniors
• Intensive Geriatric Service Workers (IGSW) - work with frail seniors after release from hospital or who display a complex number of needs
• programs for school-aged children and teens
• specialized programs for eating disorders and 1st Episode Psychosis
• family support options for families caring for children with developmental or physical disabilities - can help families apply for funding to receive one-to-one support; respite services; access to Supporting Kids in Camp program
• infant and pre-school support services - pre-school assessment consultation for child care providers and families of children 2-6 years of age

• 1st Step Program - Early Psychosis Treatment Team - Interdisciplinary team provides support to individuals in the early stages of psychosis and their families; initial assessment includes medical screening, medical history and psychiatric interviews; treatment may include counselling, medication, individual and group treatment, family education and finding supports for day-to-day living

Community based mental health services supporting people of all ages. Services include:

• adult psychiatric and mental health assessments for individuals experiencing mental health difficulties; treatment options include brief individual and group therapies, education and support groups; prescribing and monitoring of medication
• seniors specialized psychiatric and mental health assessments and treatment
• support for families with children who are experiencing Attention Deficit Hyperactivity Disorder (ADHD) or eating disorders
• Preschool Assessment Consultation Services
• Developmental services include Infant and Pre-School Support Services; Special Needs Child Care; Family Support Options, Applied Behaviour Analysis (ABA) Services, and Behaviour Therapy

Specialized service and treatment for individuals who have behaviours related to a diagnosis of dementia. Program provides service to Local Health Integrated Network (LHIN) 4 (the regions of Niagara, Brant, and Halton). Program goal is to return clients to an appropriate care setting within their home community after assessment and development of a treatment plan.
