Harvey Woods Lofts includes 80 independent apartments with staff support.

Blossom Park Townhomes consist of 27 one-bedroom townhouse-style apartments and tenants have access to housing support staff daily.

Blossom Park Apartments have 34 fully furnished one-bedroom apartments and tenants receive medication support, a daily hot meal, and additional supports from an interdisciplinary team made up of nurses, psychosocial, addiction, and housing support workers, and food security staff.

COVID19 - continuing to support the most vulnerable community members impacted by dementia through regular support telephone calls and email for those who have email, face-to-face meetings including home and office visits. Reaching out to the community by initiating support calls and monitoring office telephones daily. If emotional support is required as a result of living with or caring for someone with dementia to contact the Alzheimer Society Huron Perth at 519-271-1910 or EMAIL.

Community Education Program includes:

Heads Up for Healthier Brains - for grade 4 students and older content supports grade 4/5 curriculum on physical development and organ function. Topics include Alzheimer's disease and its effect on families, overall brain function and taking care of one's own brain health. Secondary school programs are appropriate for anatomy, family studies, parenting, biology and health classes. All programs can be tailored to meet the needs of the students and teacher

Aging with Vitality - for service clubs and community groups that focuses on reducing the risk of dementia

Dementia-related topics appropriate for service clubs, community groups, church groups and professional groups. Topics are tailored to meet the needs of the group participants.

An accredited community-based children's mental health centre that provides timely access to assessment and treatment services for a wide range of mental health concerns for children and youth up to their 18th birthday. Offers individual, group and family counselling; brief intervention; counselling and therapy; targeted prevention; intensive services. Offers parent and caregiver information and support. Services include Youth Justice, School Based Outreach and Consultation Services for Professionals. Counselling for women who have been victims of abuse - Renascence.

New Horizons Youth Engagement Program for youth mental health advocacy group for 14-24 or parents/caregivers. Call 519-273-3373.

Crisis Response - 24/7 service for serious mental health crises, suicide risk or psychosis offered in partnership with the Huron Perth Helpline and Crisis Response Team. To access call 1-888-829-7484.

Outreach Eating Disorder Program provides:
The Eating Disorders Program provides treatment for presenting concerns related to:

• Anorexia nervosa
• Binge eating disorder
• Disordered eating behaviours

Therapy is provided by a Registered Social Worker. In affiliation with the London Health Sciences Centre (LHSC) and Child and Adolescent Mental Health Care Program at LHSC.

Advocates and provides programs and services for people with mental disorders in order to enhance, maintain and promote the mental health of all individuals and communities in Huron and Perth Counties Case Management: Client directed Quality of life through community integration Ongoing support May include community connections, housing, financial planning, income budgeting, family relationships, and education about mental illness Concurrent Disorders Case Management - Case Managers provide assessment and links to treatment and rehabilitation services for people with mental illness addicted to drugs and alcohol Family Support Group: Helps families understand mental illness Offers time limited one-to-one support for family hoping to connect a loved one to mental health services Family support groups available Court Support Case Management: provides assistance to people with mental illness who come in contact with the law. Peer Support Program: people with mental illness are supported to develop peer friendships through group meetings which offer social and recreational activities. Supported Housing Program: affordable rental unites in Huron and Perth Counties, rental subsidy available based on income and evidence of a mental illness. Community Homes for Opportunity (CHO): supportive housing program for people with serious mental illness Education: offers workshops, resource centres at both office location and educational presentations to help overcome the stigma of mental illness.

Mental health and addictions team providing mental health wellness services that are culturally safe and trauma-informed for members of the urban community who self-identify as having Aboriginal ancestry

Child and Youth Services - working with children and youth 6 years of age to 24 years of age. Also work with parents, families, caregivers and child protection professionals in providing culturally safe mental health services.

Adult Services - available to individuals 25 years of age and older experiencing mental health issues / psychosocial stressors that are interfering with mental wellness

• assisting with Navigating the Health Care System
• navigation/accessing services and programs
• linkage to specialized services for Mental Health & Addictions
• accompany youth to their medical appointments (upon request)
• assist with social determinants of health that impact an individuals mental health (i.e. identification, housing, food security, life skills)
