Provides psychosocial rehabilitation services (recovery strategies) to adults with mental health issues

Provides acute services which include emergency psychiatric assessments through the Emergency Department, admission into a 60-bed acute mental health program, or admission into the 8-bed Psychiatric Intensive Care Unit.

The OPP MHRU is a collaborative program put forth by the Ontario Provincial Police and the Community Crisis Centre at Hôtel-Dieu Grace Healthcare to address the gap in the continuum of existing crisis services.

• formed to provide an integrated community based response to the target population
• vulnerable individuals who have a mental health related illness and /or a substance abuse problem who are in severe distress and often lack supports in the community
• the goal of the program is to divert these target groups from the criminal justice system and /or the hospital emergency department whenever possible by facilitating access to community services and supports.

To provide supported subsidized housing for individuals who are homeless or at risk of becoming homeless and who have a mental illness. The program works with private market landlords in the community acting in a liaison capacity to assist with providing access to safe, affordable and stable housing for consumers who are receiving community support services. This program provides:

A community health centre offering primary care services for the health needs of individuals and families.

• operates under the collaborative care model where the interdisciplinary team of family doctors, nurse practitioners, and other health care professionals including RPN, dietitian, therapist, health promoter, and chiropodist, take a wholistic approach to patient care by addressing both the primary and the mental health needs of patients.
• Treatment plan/goals of care are co-created with clients
• Referrals for on-going primary, mental health and addictions are made according to the treatment needs and identified goals of the client.
• The clinical team offers care using best practice guidelines and referral processes.

Provides services to individuals who have a pre-existing developmental disability, and mental illness. The program psychiatrist assists in consultations and follow-up recommendations for treatment for individuals requiring this service.

Children and Adolescent 6-17 yrs:

• comprehensive psychological, psychoeducational, and neuropsychological assessments including:
  • learning disorders, attention deficit and hyper activity disorder, mood and anxiety disorders
  • diagnostic clarification, treatment recommendations, educational accommodations

Community-based program providing rehabilitation services to individuals who are experiencing an extended psychosis (may include schizophrenia, schizoaffective disorder, or delusional disorder).

• mental illnesses associated with older ages (dementia)
• or severe disorders in which the exact diagnosis is unclear
• 49 beds opened to admissions across three patient care units

Assertive Community Treatment program (ACT)

• community-based, recovery model of care for individuals who have severe and persistent mental illness
• reaches out to clients where and when they need help (in home/community)
• inter-professional team of healthcare workers including psychiatrists, social workers, nurses, occupational therapists, therapeutic recreation and vocational rehab therapists

• Estate Cleaning
• Specializing in hoarding & de-Cluttering and helps organizations who deal with hoarding issues
• Maintenance Follow Up Program - 2 hrs a month will help keep place organized
• Organization of clean ups
• Will transport laundry to laundromat and wash it, as requested by pest control
• Senior Cleaning Assistance
• Help with cleaning skills

Outpatient service that uses cognitive behaviour strategies to help adolescents with depression, obsessive-compulsive disorder and anxiety

• 12 to 20 individual and/or family sessions, usually on a weekly basis
• Psycho-education for both the young and family which focuses on exploring solutions, identifying resources and developing new approaches to coping with emotions, thoughts and social situations

The Regional Children's Centre is an accredited children's mental health centre working in collaboration with children, youth, and their families who are experiencing mental health and/or developmental concerns.
