Provides access to in-home health and personal support services to help individuals live independently at home

• Assist community members with the transition to long-term care homes when living at home is no longer possible
• Can provide information about, and can link individuals to, other helpful services available in the community
• Care Coordinators are health care professional specially trained to help patients investigate and organize their health care services so that patients are able to achieve the best possible state of health and well being.

Community Nursing Clinics - Nursing services for ambulatory patients at clinics located in the community

Health Care Connect - Refers people who do not have a doctor to a family health care provider who may be accepting new patients * once registered, program participants are assigned to a Care Connector employed by the Home and Community Care Support Services * Care Connectors work with family doctors and nurse practitioners who are accepting new patients

In-Home Services - Health care services to people in their own homes

Information and Referral - Provides information on services available in the community or in-home * assesses needs of new clients

Mental Health and Addictions School Program - Assessments, interventions and support to students and families with mental health and or addiction issues * consultation to school staff in regards to mental health and/or addiction issues as appropriate

Ontario Telehomecare Program - 6 month telephone coaching, support and monitoring program patients with mild to moderate Chronic Obstructive Pulmonary Disease (COPD) and Congestive Heart Failure (CHF) * equipment provided (blood pressure, weigh scale, pulse oximeter and tablet) to report on health daily * weekly discussions with telehomecare nurse

School Health Support Services - Special health services for children who require medical or rehabilitative assistance to attend school, participate in school routines and receive instruction * services are provided within public, private or home-based schools

• secured entrance to both facility and home area
• provides 24 hour professional nursing care
• facility features two distinct living areas
• private, semi private and basic bedrooms have electric beds
• semi private rooms have one bed and share washroom with one other resident
• basic rooms have two beds and share a washroom
• nurse call system
• mechanical lifts available
• social and residential programs
• facilities for couples
• activity rooms, lounges, courtyard, worship centre, special events room for residents, hairdressing salon, dental services, chiropody (foot care)

Family Transition Program
For individuals in Oxford County who are 18 years of age or older and are waiting for permanent placement into a long-term care facility of their choice. Key features include completion of a supportive community screening; ensuring required services and community supports are in place prior to the individuals admission into long-term care; and finally, a review of life and medical history, previous agency involvement, and any access needed to continue those services for the individual once they have entered their new long-term care residence.
