Ontario Telehomecare Program - 6 month telephone coaching, support and monitoring program patients with mild to moderate Chronic Obstructive Pulmonary Disease (COPD) and Congestive Heart Failure (CHF) * equipment provided (blood pressure, weigh scale, pulse oximeter and tablet) to report on health daily * weekly discussions with telehomecare nurse
School Health Support Services - Special health services for children who require medical or rehabilitative assistance to attend school, participate in school routines and receive instruction * services are provided within public, private or home-based schools
