Searching for Supportive (semi-independent) Housing

A regional supportive housing program for clients who are homeless with complex needs who require multiple supports. The program offers a multi-disciplinary team including; Mental Health Worker, Addiction support, Landlord Engagement Specialists, and Intensive Case Managers.

This program is delivered in partnership with Southridge Church. Port Colborne has a 7 apartment complex that is dedicated to individuals of the Home for Good Program.

A recovery-orientated approach to end homelessness that centers on quickly moving people experiencing homelessness into independent and permanent housing as well as providing additional supports and services as needed.

Supports offered include life skills, employment, emotional and resource linkage. A moderate level of support is provided and a crisis plan is completed once a person receives housing.

The Housing First program in Niagara is for eligible individuals. Individuals must be referred to the program from the Niagara Region's By Name List.

A non-profit, faith-based organization serving people with developmental disabilities and their families in Ontario. Supports and Services in Niagara region include residential, supported independent living, employment supports, and family home.

Provides community living alternatives through a variety of residential support options for individuals with an intellectual disability.

Services for people who have an intellectual disability including:

• preschool development
• adult vocational support
• family support
• leisure activities
• residential accommodation
• coordination with other community services to provide support to individuals and to families

Long-term residency and transitional housing accommodation in a group setting for physically disabled adults.

• 24 hour attendant care staff services assist with the activities of daily living to enable them to live as independently as possible
• Residents are expected to direct their own services
• Adult Day Program services are available for residents 5 days/week

Provides independent supportive living for individuals with mental health and developmental disabilities.

Offers the following services to residents:

• Onsite/on call staff qualified to supervise safety in the home
• Furnished rooms
• Three meals plus refreshments throughout the day
• Assistance with laundry and housekeeping (as required)

Provides short and long-term housing accommodation in group homes and supported living situations to individuals with developmental disabilities.

This program provides minimal support staffing assistance (based on need) to adults living independently in the community. The assistance includes help with budgeting, cleaning, shopping, counselling, cooking and leisure time usage.

Community-based support and rehabilitation services to adults living with the effects of an acquired brain injury * family and support persons are encouraged to participate

Group Home Living
Designed for adults with special needs who desire full time residence with a small group of their peers. Offers a variety of housing arrangements throughout the Niagara Region which includes urban and rural settings and barrier free accommodations.
Trained staff provide 24-hour support to individuals living in a family-like setting. Individuals participate individually or as a group in their community activities of their choice. Individuals may be supported by other Bethesda programs including Chaplaincy and Recreation and Leisure Services.

Familyhome Program
The Familyhome program / LifeShare provide adults with special needs a home-like setting with a sense of family, safety, continuity, and security. Promote a high quality of life and support community involvement, social inclusion, individual choice, independence, and rights and responsibilities. Through Familyhome / LifeShare, Ontario families and citizens (home providers) can offer a safe and nurturing environment for adults with special needs. The Familyhome / LifeShare Program encourages personal choice, decision making, independence, and participation in the community. Familyhome Providers are carefully screened through references and police checks, health and fire home inspections, and extensive interviews. Familyhome Providers receive 24-hour on-call support from Case Managers.

Supported Independent Living
For adults with special needs who desire to live in the community alone, or with others, but independently of family members or a caregiver. Provides supports fordaily living, as needed, and training related to money management, banking, using public transportation, and other life skills.The SIL program encourages personal choice, decision making, independence, and participation in the community.

Respite Services
For adults with special needs who live at home with their families. Offers weekend activity-based respite in an eight-bedroom home setting in Vineland. Participants enjoy a variety of recreational opportunities including crafts, games, music, picnics, swimming, bowling, etc., based on the individual's interests, strengths and needs. During the summer months and periodically throughout the year, respite services are offered weekly and individuals may utilize this service for up to two weeks. Special conditions apply. Advanced planning is essential and regular, planned respite is recommended as space is limited

Offers affordable housing (independent living) with on-site support that may provide advocacy services, information and referral, and connection to community services, social and recreational activities.

A Community Room provides space for both tenants' programs and a meeting space for other non-profit groups.

Some units are accessible and designed for persons with disabilities.

Offers a variety of social, wellness and outreach services to older adults 60+ who reside in Ontario and have a valid Health Card. Programs are intended to help older adults remain safe and independent at home for as long as possible. The following non-medical programs are offered to support seniors and their families and caregivers. Programs include group sessions, in-home services and online programming.

Rent-geared to income housing and support for adults with a developmental disability and/or dual diagnosis. Support can range between 2 hours per week and up to 24 hours per day based on individual need.

Supportive Housing with attendant services to the tenants * a furnished respite apartment also available for physically disabled adults in the Niagara Region who require temporary attendant services with non-medical activities of daily living.

A residence for individuals with a mental illness. Provides meals and 24 hour staffing. Nursing care is not available. The home is licensed for 15 residents. Homes for Special Care provides a supervised setting for psychiatric patients of St Joseph's Health care Center for Mountain Health Services and are licensed under the Ministry of Health. The Center works with the Homes and provides supportive services to the clients. There is no time restriction to length of stay.
