• Free milk coupons
• Follow up via telephone within 48 hours of discharge from hospital
• Home visits pre birth to prepare family for new arrival
• Home visits post birth to offer support to new mothers
• Home visits during child's early years to offer development guidance
• Referrals to other programs and services

Also offers:

• Educational and promotional workshops and presentations to schools, community groups and parents
• Hosts cultural events

Provides supports and services to help families improve maternal and infant health and well-being with a focus on the following three areas:

Maternal Nourishment that may include:

• Baby food making and nutritional cooking sessions
• Healthy snacks and meal information
• Milk and food vouchers as well as vitamin supplements

Nutrition Screening, Education and Counselling includes:

• Education on prenatal nutrition
• Understanding food labels
• Presentations by dietitians and health specialists
• Nutrition screening by Community Health Representative
• Individual counselling by nutritionists

Breastfeeding Program includes:

• Promotion of breastfeeding initiation and duration
• Support groups
• Breastfeeding videos and resource materials

Provides nutrition education and information to expectant and new mothers

Programs include:

• Breast-feeding
• Caring for your baby
• Labour and birth
• Healthy lifestyle
• Nutrition
• Prenatal care
• Your pregnancy

Due to COVID-19, the program is offered virtually

Connects individuals with a nurse and/or a Lamaze instructor

Provides support and information on:

• prenatal care
• labour
• birth
• care of the newborn
• breastfeeding

Visits can be arranged at either the McKenzie Street site or Westfort Family Resource Centre site, or in an individuals home

Delivers a voluntary parenting support program and promotes healthy child development

Program participants are provided with:

• Support and education during pregnancy through referral and home visiting as appropriate
• Follow up via telephone within 48 hours of discharge from hospital
• Home visits and referrals post birth to offer support to new mothers as required
• Home visits during child's early years to offer development guidance as required
• Referrals to other programs and services

Sexual health clinic

Provides a variety of sexual health services, including:

• Sexually Transmitted Infection (STI) testing and treatment
• Birth control purchase/depo shot (with a valid internal prescription on file)
• Pregnancy testing, prenatal referrals and options counselling
• PAP testing
• Emergency Contraceptive Pill (Plan B)
• Referrals related to sexual health issues, including abortions

Anonymous HIV testing

Provides quick and private HIV testing, in which the name or identity of the person being tested is not requested, recorded or reported; the test is ordered using a code known only to the person being tested

Offers the following services:

• HIV information and testing
• Outreach services and education
• Referral to community services
• Pre-test counselling on how people get HIV, and what a positive or negative test result might mean
• Post-test counselling to discuss the results

Provides individual, family and community health programs and services through information sessions, clinics, screenings, home visits and a spectrum of educational tools

• Offered at Hope Place, the HOPE for Mothers group offers support for mothers looking to make change in their drug or alcohol use
• Individual and group support for women who are pregnant or parenting and struggling with substance use, mental health, abuse, and child protection issues
• Teaches parenting skills and healthy lifestyle choices, self-care and well-being, relapse prevention and life skills

• Delivers a parenting support program and promotes child development and healthy living
• Provides support through family home visits, service coordination, and referrals

Supports for people living with prediabetes or diabetes

• Provides the knowledge and skills for people with diabetes to take control of their health
• Designated as an Assistive Devices Program registered for the provision of insulin infusion pump training in Northwestern Ontario

Pediatric Diabetes

• Offers education and clinic services to families of children and youth with diabetes
• Provides necessary survival skills to allow the patient to manage their diabetes at home
• Provides ongoing medical care and support
• Offers educational and social events, including support groups

Pregnancy Program

• Provides education in meal planning, blood glucose monitoring, use of insulin pens, and recording results
• Offers preconception counseling to ensure good control of diabetes before pregnancy
• Provides follow up during the pregnancy as well as post-partum, providing further support as required

Type 1 and Type 2 Diabetes in Adults

• Provides education and support as well as access to the tools and resources to help clients self-manage

Prediabetes Program

• Teaches individuals who are at risk for diabetes the skills to help them to prevent or delay diabetes
• Provides information in a group setting on topics such as activity, cholesterol, weight management, and healthy eating

Delivers a parenting support program and promotes healthy child development

• Provides group information and educational sessions on topics about healthy pregnancy, birth and taking care of baby at home
• Provides information on the stages of pregnancy

Baby and Me

• Provides group support through weekly talking and sharing circles
• Encourages relating personal experiences
• Offers guest speakers occasionally to talk on related topics

Breastfeeding Program

• Provides information and instruction concerning appropriate techniques for nursing an infant for expectant parents or new mothers
• Loans out breastfeeding pumps and other related equipment

Cooking Together

• Provides a weekly lunch program; families plan, prepare and eat a healthy meal together
• Provides grocery budgeting and shopping advice

Fruit Basket

• Provides fruit/vegetable vouchers and vitamin supplements to enhance healthy pregnancy
• Provides child care for one hour per week for any reason
• Provides an informal afternoon of socialization, information sharing and to pick up fruit baskets and vegetable vouchers

Provides prenatal and postnatal care for pregnant women and new mothers

• Offers prenatal education to all pregnant women
• Conducts prenatal home visits
• Works under a medical directive which allows the necessary lab work orders to be placed right from home visit

Services include:

• Weighing patients
• Urine analysis
• Blood pressure checks
• Postpartum education
• Lactation advice
• Loan of hospital grade electric breast pump
• Weighing and assessing newborns
• Education to new moms about newborns
• Referrals and information about programs and services available in community
