Complex Mental Health and Addictions - includes mental health conditions with severe effects on client functional abilities. For all clients with ongoing addiction concerns, this team will provide the most wraparound care and support.

Social Work Support Program - provides confidential short-term, and goal-focused counselling to help individuals dealing with Depression, Family Conflict, Anxiety, Stressful life situations, Relationship issues, using Cognitive Behavioural Therapy, Rational Emotive Therapy, Assertiveness Training and Solution Focused/ Brief Therapy principles. Access to social work is through referral from your CCHC Physician or Nurse Practitioner.

Anger Solutions Program™ helps individuals develop appropriate ways to express and resolve their anger. Built on a psychosocial rehabilitation foundation, Anger Solutions incorporates the best of several evidence-based therapeutic models.

Chronic Disease Management (CDM) and Older Adult - includes clients whose primary diagnoses are diabetes, COPD, cardiovascular conditions, etc. who require routine monitoring and will benefit from support in self management of these conditions. This includes all clients who are 65 years and over as many will overlap with the CDM conditions, thus we also routinely monitor osteoporosis, falls risks and cognitive changes.

Osteoporosis program: Watch for the upcoming launch of our new bone health and falls prevention program! Clients will have the opportunity to learn about their risk level for osteoporosis, eating well for strong bones, fall prevention techniques, appropriate exercises and medication education.

Physiotherapy Program - The Physiotherapist performs assessment, diagnosis, treatment, health education/counseling. Various treatments are prescribed including but not limited to, balance exercises, transfer training, range of motion, strengthening, application of modalities. (i.e. heat, ice, ultrasound, TENS). Access to physiotherapy is through referral from your CCHC Physician or Nurse Practitioner.
Seniors' Social Group -provides seniors with an opportunity to get out and meet new people, enjoy music, snacks, and fun activities in a relaxed atmosphere.

Smoking Cessation Program - Provides high quality one-on-one smoking cessation behavioural counselling, coaching and support to clients wishing to reduce or quit smoking, including assessing client readiness to reduce/quit smoking, review of evidence-based pharmacotherapies and psycho-social interventions, assisting client with development of a Quit Plan, and coaching, supporting and encouraging client in the achievement of their Quit Goals. Access to the Smoking Cessation Program is by referral from your CCHC Physician or Nurse Practitioner.

Preventative/Prenatal/Pediatrics - this group will primarily be clients who need mostly screening programs, immunizations, episodic care, supportive education, and programs to support healthy lifestyles.

Nutrition program - The CCHC Dietician performs community nutritional needs assessments and identifies barriers to acquiring adequate food security, facilitating and supporting community initiatives to improve food security and performs nutritional assessments, development of client-focused nutritional plans, health education/counseling. Access the Dietician is through referral from your CCHC Physician or Nurse Practitioner or from the community at large.

Craving Change^TM - Watch for the upcoming program that provides cognitive-behavioural tools, activities and strategies that address the universal struggle to change problematic and emotional eating behaviours. The thought-provoking, 'how-to' approach focuses on the 'why' of eating behaviour and what to do about it.

St. Thomas Outreach program provides care to those who are homeless or underhoused or face other barriers to care, such as not being able to access a family physician or nurse practitioner, lack of Ontario health insurance benefits or lack of transportation.

Mobile Unit Outreach brings care to primarily uninsured folks in the county.

The Province has a Testing and Isolation Information Line that can be used for information regarding testing and access, confirmed or suspected COVID-19 cases, self-isolation, or exposure to COVID-19 1-888-777-0730.

COVID19 Offices open. Call for intake and protocols. Clients will be supported either in-person, over the phone or via videoconferencing. Intake and main office lines will be monitored and messages responded to within one business day. Some peer support groups are running via Zoom. Ensuring clients have resources they need to stay connected and safe. Still taking referrals, please call.

The Addiction Medicine Clinics supported in partnership with Dr. Datema, are considered an essential service so will remain open on their usual days. They are doing active telephone screening day before clinic and active screening is taking place at clinics and practicing social distancing by limiting the number of clients at the clinic at one time.

Programs offered:

19 Day Residential Chemically Dependent Program
Highly structured program includes individual counseling, videos, discussions, lectures and recreation, a six month Recovery Support program follows residential treatment. The program uses a variety of methods to help clients gain an understanding of addiction as a primary illness. Help in recognizing the thinking patterns that have established the need for substances. Make a commitment towards accepting personal responsibility for maintaining abstinence in recovery. Life Skills help for a renewed value system.

7 Day Residential Co dependency Program
Open to men and women who feel their life has been affected by someone else's drinking or drug abuse. Program is offered bi-weekly. A 6 month Recovery Support program follows treatment.

Weekend Family Program
For family members of clients presently in the residential addiction program. The family programs provides information on substance abuse and how it affects the family members (through lectures, group discussions and videos. Guidance is given on changes individuals can make to help them deal with the affects of substance abuse on them, and their subsequent reaction to life situations. Due to Covid-19 this program is not running for family members at this time

Stage Two Relapse Prevention Program
A 6 day Residential program is offered 6 months or longer after attending residential addiction treatment

The Addiction Assistance Service
Operates 24 hrs a day and will aid with finding support for a persons immediate addiction problems from the community services available such as:

• Full withdrawal from substance abuse at the Windsor or London Withdrawal Management Centres
• Counselling for addiction problems.

NEW: Virtual Day Program
An outpatient virtual addiction treatment program for those looking for flexible care or those not suited to or unable to attend residential treatment. The program is open to men and women 16 years of age and older.

Addiction Program - assessment and outpatient counselling for people of all ages experiencing issues with alcohol, drug use and gambling. Mental health and peer support also available. Assistance is available to family and friends. Additionally, counsellors are available through the guidance offices of high schools. Community education is a major component of this program

Adult Mental Health Program - community-based, client-focused services to adults with mental illnesses or with concurrent disorders (mental illness and addictions) including assessment, consultation, treatment, education, support, and advocacy. It is an interdisciplinary staff (physicians, nurses, social workers, with support from addiction counsellors and peer support as required)

Specialized Geriatric Services - provides non-emergency clinical assessment, consultation, treatment and education to older adults, their families, and service providers who are, or know of, someone who is experiencing mental health issues combined with age-related difficulties
