Offers a snow removal service for qualified seniors and persons with disabilities. The service consists of snow removal from city sidewalks fronting and abutting the homes occupied by qualified seniors and persons with disabilities. It is not for the removal of snow from snow plow windrows, driveways or any walkway to a home.

Applicants must register for the program by the end of November each year.

Provides grants of up to $3,500 to homeowners who have or are at risk of having a sanitary back-up as a result of sewer surcharging.

FLAP consists of a grant to offset the cost of eligible work, which may include:

• The installation of a backwater valve to stop sewage back-ups
• The installation of a sump pump with a back-up pump (battery or water-powered)
• The disconnection of weeping tiles from the sanitary sewer
• The re-connection of these weeping tiles to the sump pump
• All materials and labour associated with the installation of the protective devices

Offers residential and lifestyle supports including a wide range of personalized supports to people living in the community - primarily outside of their family home.

Many continue to receive support within group home settings, others reside in home-share situations or apartments.

People who access services receive supports only as needed to encourage their independence.

Supports may range from direct personal care * health care * housing help and skill building to a consultative model of support for those people involved in the Supportive Living Program.

Health related and/or practical services to people living with or at risk of HIV, HepC or STBBIs who need assistance and wish to remain in a homelike environment. Case management, personal care and social supports are provided.

COMMUNITY SUPPORT PROGRAM provides support to people who have serious, persistent mental illness, who are living in the community in the setting of their choice and who, at times, require support to maintain their housing and independence. This program provides low to moderate intensity support based on individual needs.

SUPPORTIVE HOUSING PROGRAM is designed for individuals requiring long-term intensive support and who are experiencing serious mental health issues. These individuals are homeless or at risk of becoming homeless. The program provides a combination of affordable housing and the ongoing, flexible supports needed to maintain housing. Consumers are linked with services, supports and resources in the broader community. The program supports over 80 housing units in the Niagara Region.

TRANSITIONAL HOUSING SUPPORT PROGRAM: provides therapeutic support to clients with concurrent issues. Life skills instruction and practice is an integral part of the program. The program is time limited to a period of 3 to 6 months. Assistance with finding permanent housing is a component of this program.

Services offered span a full range of both medical and non-medical home care for seniors consisting of three pillars of caring:

Taking Care: companionship * outings and community experiences * meals * light housekeeping * appointment escort * physical activity * groceries and errands

Enriched Care (PSW): bathing, dressing and toileting support * meal preparation and assistance (if required) * ensure home safety * medication management (and reminders) * Alzheimer's and dementia support

Vital Care (Nursing): daily, live in, or end-of-life caring * Alzheimer's and dementia support * live-in caring * wound care/post-operative caring / complex caring * special needs caring

CSSN staff member provides an individualized home assessment and summary of recommendations to promote independence and mobility. Referrals to other services can be made when required.

CSSN screens and makes referrals to brokered workers for odd jobs, minor repairs, yard work, heavy housecleaning and decluttering, and snow removal, etc.

Clients are provided with referred worker's contact information and are responsible for arranging service(s).

Follow-up calls are made by the agency to ensure reliable and quality services are provided.

Home-workers provide light housekeeping, laundry, occasional meal preparation, and grocery shopping. The same home-worker is assigned to a client on a regular and consistent schedule. Services are available on weekly, bi-weekly, or monthly basis for a minimum of two hours. Client must supply all cleaning products and equipment.

Note: Housekeeping services are intended as maintenance cleaning. The program does not include deep cleaning.

Personalized and compassionate care through quality health services, to enhance the well-being of people at vulnerable times in their lives with an acute or chronic condition as a result of a physical or mental illness (inclusive of palliative and respite care). Personal care, light housekeeping, meal prep, respite and companion care to residents throughout the Niagara/Burlington areas.

Nursing staff provides visiting in the community including in the home, school, or clinic setting.
