Many continue to receive support within group home settings, others reside in home-share situations or apartments.

People who access services receive supports only as needed to encourage their independence.

Supports may range from direct personal care * health care * housing help and skill building to a consultative model of support for those people involved in the Supportive Living Program.

Offer support services, programs and resources to at risk youth. Services include:

Emergency Shelter: Youth are given access to a bed, showers and meals and also programming including the Steps to Independent Living program, recreation and social activities and cooking programs. Also, offers emergency shelter to victims of human trafficking as well as crisis support and referrals.

Drop-in Centre: A place for youth to connect with peers and receive mentoring and support. Youth will have access to meals, phones and Wi-Fi, shower and laundry facilities, crisis counselling and the clothing closet and food cupboard. Youth will also have access to the Steps to Independent Living program and recreational programs.

Youth Reconnect: Connects individuals in unstable living conditions with the services they need without them having to leave their home community. Referrals can be made by anyone and a Youth Worker will contact you within 24 hours.

After School Programs: In partnership with over 15 different service providers, the RAFT operates six After School Programs for youth. Programs offer free and accessible recreation, leisure and educational opportunities.

Youth in Transition: Supports youth, ages 16-24, in their successful transition out of the child welfare system into adulthood. Workers assist help youth to develop and pursue their goals, and will support them to identify access and navigate adult service systems relevant to their specific needs.

Eternal Routes: Connects homeless youth unable to live with their parents and Crown wards aging out of foster care with relatives who would be willing to provide the youth with the support they need to transition into adulthood and avoid homelessness.

Niagara Assertive Street Outreach: Actively engages individuals experiencing unsheltered homelessness to promote, connect, and coordinate access to the range of supports available across Niagara's Homeless-Serving System, including emergency shelter, housing and support services.

A residence for individuals with a mental illness. Provides meals and 24 hour staffing. Nursing care is not available. The home is licensed for 15 residents. Homes for Special Care provides a supervised setting for psychiatric patients of St Joseph's Health care Center for Mountain Health Services and are licensed under the Ministry of Health. The Center works with the Homes and provides supportive services to the clients. There is no time restriction to length of stay.

COMMUNITY SUPPORT PROGRAM provides support to people who have serious, persistent mental illness, who are living in the community in the setting of their choice and who, at times, require support to maintain their housing and independence. This program provides low to moderate intensity support based on individual needs.

SUPPORTIVE HOUSING PROGRAM is designed for individuals requiring long-term intensive support and who are experiencing serious mental health issues. These individuals are homeless or at risk of becoming homeless. The program provides a combination of affordable housing and the ongoing, flexible supports needed to maintain housing. Consumers are linked with services, supports and resources in the broader community. The program supports over 80 housing units in the Niagara Region.

TRANSITIONAL HOUSING SUPPORT PROGRAM: provides therapeutic support to clients with concurrent issues. Life skills instruction and practice is an integral part of the program. The program is time limited to a period of 3 to 6 months. Assistance with finding permanent housing is a component of this program.

Supportive Housing with attendant services to the tenants * a furnished respite apartment also available for physically disabled adults in the Niagara Region who require temporary attendant services with non-medical activities of daily living.

87 assisted living suites (studio, deluxe studio, one bedroom suites) * provides short stay ( temporary, vacation, convalescent, respite care) when space permits. Minimum 30 day stays.

Rent-geared to income housing and support for adults with a developmental disability and/or dual diagnosis. Support can range between 2 hours per week and up to 24 hours per day based on individual need.

Radiant Care is a private, not-for-profit organization providing accommodation and care to senior citizens.

Offers wellness suites, which are semi-independent living apartments with care and support available. Supports may include bathing, lifting, transferring, medication reminders, light housekeeping and laundry, and 24 hour safety checks. Wellness suite tenants are provided with meals and access to a 24-hour emergency call bell system.

Free Exercise and Falls Prevention classes are offered on-site. Tenants and local seniors also have access to an on-site Fitness and Wellness Centre.

Supportive Housing with 12 units offering 24-hour attendant service

Offers a variety of social, wellness and outreach services to older adults 60+ who reside in Ontario and have a valid Health Card. Programs are intended to help older adults remain safe and independent at home for as long as possible. The following non-medical programs are offered to support seniors and their families and caregivers. Programs include group sessions, in-home services and online programming.

Group Home Living
Designed for adults with special needs who desire full time residence with a small group of their peers. Offers a variety of housing arrangements throughout the Niagara Region which includes urban and rural settings and barrier free accommodations.
Trained staff provide 24-hour support to individuals living in a family-like setting. Individuals participate individually or as a group in their community activities of their choice. Individuals may be supported by other Bethesda programs including Chaplaincy and Recreation and Leisure Services.

Familyhome Program
The Familyhome program / LifeShare provide adults with special needs a home-like setting with a sense of family, safety, continuity, and security. Promote a high quality of life and support community involvement, social inclusion, individual choice, independence, and rights and responsibilities. Through Familyhome / LifeShare, Ontario families and citizens (home providers) can offer a safe and nurturing environment for adults with special needs. The Familyhome / LifeShare Program encourages personal choice, decision making, independence, and participation in the community. Familyhome Providers are carefully screened through references and police checks, health and fire home inspections, and extensive interviews. Familyhome Providers receive 24-hour on-call support from Case Managers.

Supported Independent Living
For adults with special needs who desire to live in the community alone, or with others, but independently of family members or a caregiver. Provides supports fordaily living, as needed, and training related to money management, banking, using public transportation, and other life skills.The SIL program encourages personal choice, decision making, independence, and participation in the community.

Respite Services
For adults with special needs who live at home with their families. Offers weekend activity-based respite in an eight-bedroom home setting in Vineland. Participants enjoy a variety of recreational opportunities including crafts, games, music, picnics, swimming, bowling, etc., based on the individual's interests, strengths and needs. During the summer months and periodically throughout the year, respite services are offered weekly and individuals may utilize this service for up to two weeks. Special conditions apply. Advanced planning is essential and regular, planned respite is recommended as space is limited

Community-based support and rehabilitation services to adults living with the effects of an acquired brain injury * family and support persons are encouraged to participate
