Operates as a program under the Healing and Wellness Branch

• Provides information, advocacy and support to assist individuals to remain in their homes as long as possible
• Acts as respite for informal caregivers such as extended family

Community Support Services offered include:

• Caregiver support
• Case management
• Friendly visiting
• Service arrangement and coordination
• Transportation for medical appointments within Sault Ste Marie and/or assistance with medical appointment arrangements
• Visiting for social and safety reasons, either in person or on the phone

• Offers a designated driving service that will also drive the client's vehicle from one point to another

Offers the following transportation services:

• Airport drop-off/pickup
• Chauffeur and valet service
• Designated driver (drive vehicle home as well)
• Specialty transportation includes outpatient and assisted transportation services
• Wedding and events transportation

Offers assistance to patients who are transitioning from a hospital stay to home which may include the following:

• Assistance with transportation
• Home configuration
• Shopping
• Finding other community support services to help

Provides affordable door to door transportation for seniors or those with a disability to help them with various activities such as grocery shopping, medical appointments and social outings

NOTE: Vehicles are not equipped to accommodate wheelchairs

Delivers medical transportation services and assistance in making medical appointment arrangements for community members

• Provides transportation to medical appointments within the community
• Provides transportation from/to Batchewana First Nation Reserves to medical appointments (doctor, dentist, optometrist, specialist) in Sault Ste Marie and other communities
• Provides assistance with making arrangements for any medical appointments out of town such as booking appointments, hotels, and completing applications for mileage compensation

Responsible for processing Non-Insured Health Benefits Service claims and coordinating medical transportation for community members

• Provides transportation within the community for individuals accessing the Nursing Station
• Submits medical appointment travel, mileage, meal and accommodation requests on behalf of individual
• Provides assistance with completing prior approval applications for long distance medical appointments
• Offers assistance with arrangements for accommodation and travel details for individuals attending medical appointments at Sioux Lookout Meno Ya Win Health Centre

Delivers medical transportation services and assistance in making medical appointment arrangements

• Provides transportation to medical appointments within the community
• Provides transportation from/to Garden River First Nation Reserve to any medical appointments (doctor, dentist, optometrist, specialist) outside the community
• Provides assistance with making arrangements for any medical appointments out of town such as booking appointments, hotels, and completing applications for mileage compensation

Delivers medical transportation services and assistance in making medical appointment arrangements

• Provides transportation to medical appointments within the community
• Provides transportation from/to Thessalon First Nation Reserve to medical appointments (doctor, dentist, optometrist, specialist) outside the community, usually in town of Thessalon, Blind River or Sault Ste Marie
• Provides assistance with making arrangements for any medical appointments out of town such as booking appointments, hotels, and completing applications for mileage compensation

Delivers medical transportation services and assistance in making medical appointment arrangements

• Provides transportation to medical appointments within the community
• Provides transportation from/to Mississauga First Nation Reserve to any medical appointments (doctor, dentist, optometrist, specialist) outside the community
• Ensures that community members are informed of any changes in appointments
• Supervises the duties of the medical transportation drivers
• Provides assistance with making arrangements for any medical appointments out of town such as booking appointments, hotels, and completing applications for mileage compensation

Provides volunteer transportation to medical appointments, both within Elliot Lake and outside of the city

Delivers medical transportation services and assistance in making medical appointment arrangements

• Provides transportation to medical appointments within the community
• Provides transportation from/to Serpent River First Nation to medical appointments (doctor, dentist, optometrist, specialist) outside the community, usually in Blind River, Sudbury or Sault Ste Marie
• Provides assistance with making arrangements for any medical appointments out of town such as booking appointments, hotels, and completing applications for mileage compensation

Promotes the development of good mental health and provides services for individuals with mental health issues to enrich their quality of life

Provides counselling, information and awareness activities including but not limited to:

• Abuse and addictions
• Anger issues
• Childhood trauma
• Depression
• Family violence
• Grief or loss
• Loneliness
• Stress management

Services can include:

• Advocacy including accompaniment to appointments
• Aftercare, offered on a voluntary basis for persons returning from treatment centres or abstaining on their own
• Assessments
• Counselling services for stress management and crisis intervention
• Prevention services
• Referrals to appropriate supports
• Transportation to attend mental health/addictions appointments

Psychiatric Services are NOT provided by the Mental Health Unit but available through Brian Nootchtai at the North Shore Tribal Council (NSTC), or the East Algoma Mental Health Clinic

• Provides home support to Indigenous seniors so they can remain in their own homes as long as possible
• Provides education and information related to health and well-being
• Acts as a resource and respite for family members who are caring for an elderly loved one

Services provided can include but are not limited to the following:

• Assistance with personal care such as bathing
• Home support with light housekeeping, laundry and meal preparation
• Friendly visits to help with loneliness
• Assistance with coordination of contracted services for lawn care and snow removal
• Transportation/accompaniment to medical appointments and for errands such as banking and grocery shopping

• Maintains client documentation, receipts and related information
• Provides transportation to medical appointments within the community and medical appointments out-of-town in Espanola, Sudbury and other centres

Responsible for processing Non-Insured Health Benefits Service claims and coordinating medical transportation for community members

• Provides transportation to medical appointments within the community for members without access to a vehicle
• Provides transportation and/or assistance from/to Aundeck Omni Kaning First Nation to medical appointments (doctor, dentist, optometrist, specialist) in Little Current, Espanola and surrounding area
• Submits medical appointment travel, mileage, meal and accommodation requests on behalf of individual
• Provides assistance with making arrangements for any medical appointments out of town such as booking appointments, hotels, and completing applications for mileage compensation

NOTE: Individuals with small children must have appropriate car seat restraints or transportation will not be provided
