A Not-for-Profit community committee that has been organized to address the shortage of physicians and healthcare workers in our community. Call this office if attempting to find a doctor.

Dedicated to enhancing the health, quality of life and independence of individuals in our communities by offering a single point of access to home and community services. Our services include: case management, specialized nursing programs, nursing clinics, personal support services, occupational therapy, physiotherapy, social work, speech-language pathology, information and referral to community health and social services, services for children and youth, palliative care consultation, telehomecare and much more.

Health care services offered at home, at school, and in the community, as well as access to supported living programs and long-term care * care coordinators work with people in the community or leaving the hospital to determine what help is needed and arrange for care
Services may include:

• nursing
• nurse practitioner
• personal support
• physiotherapy and occupational therapy
• speech and language therapy
• social work
• nutrition and food counselling (dietitians)
• palliative care
• medical supplies and equipment
• access to long-term care homes
• access to supported living (adult day programs, supportive housing, retirement homes and assisted living)

• specialized services for people living with specific health conditions

Dental services covered under this program will include the following:

• Examinations/assessments
• Preventive services (e.g., cleanings)
• Restorative services to repair broken teeth and cavities
• X-rays
• Oral surgery services to remove teeth or abnormal tissue
• Anaesthesia
• Endodontic services to treat infection and pain
• Periodontal services to treat gum conditions and diseases
• Prosthodontic services, including dentures, will be partially covered

• refers the general public to doctors who are accepting new patients
• handles complaints brought against physicians

Supports solo practicing Primary CareProviders (PCP) who provide team-based health care to their patients helping to meet their health and wellness goals. Individuals are referred to the program by their PCP.

Refers people who do not have a doctor to a family health care provider who may be accepting new patients * once registered, program participants are assigned to a Care Connector employed by the Home and Community Care Support Services * Care Connectors work with family doctors and nurse practitioners who are accepting new patients

The Huron Perth Ontario Telemedicine Network (OTN) Primary Care Nurse Pratitioners provide primary care service to:

• Individuals with mental health issues and/or addictions
• Complex frail elderly individuals

This is a temporary care service for individuals without a family doctor to provide support until they have a family doctor. Care is delivered in person or via OTN that uses secure video technology

Provides access to community care, links people to a wide range of community services and supports, care coordinators work with patients/clients in community and hospital settings to determine the right care and support they require, care is delivered to the client in various community settings including home, school, residential facility and at flex clinics

Services available to eligible clients may include:

• Nursing
• Nurse practitioner
• Physiotherapy and occupational therapy
• Nutritional Counselling/Dietetics
• Speech-Language therapy
• Social work
• Personal support
• Medical supplies and equipment
• Connecting to other community support services and agencies
• Mental Health and Addiction Nurses (MHAN)

• Access to assisted living and supportive housing
• Access to respite care and convalescent care (short-term stays and adult day programs)
• Access to long-term care homes, rehabilitation and complex continuing care beds
• School health support services to school aged children
• Community support for people living with cancer
• Hospice Palliative Care
