Aim is to increase public awareness for the need for organ donors, to provide support for people on waiting lists, and to offer support to other organ recipients who may be experiencing difficult times because of medications, guilt feelings, etc. Services are offered to assist organ recipients and their families with medical and other costs related to the transplant operation in Sarnia and Lambton County.

COVID-19 - Offices are closed, to the public, however, all support activities are being conducted.

• Special Services at Home - a provincial program that helps children and youth with an intellectual disability, and children with a physical disability and their families or guardians by paying for special services they need to live at home and to enjoy learning and participating in the community. Funds can be used to teach new skills or abilities and to provide respite support to the family or guardian.
• Passport - assists adults with a developmental disability and their families or guardians to organize meaningful, individual and group activities that respond to the needs and aspirations of those supported and purchasing services through this program.
• Family Managed Respite - enables a wide range of self-directed funding providing family members or guardians with an opportunity to have periodic breaks from the daily responsibility of care giving.
• Out of Home Respite - extends funding for families or guardians to purchase planned and coordinated respite supports for their children with complex multiple needs. Out of Home Respite is a respite service provided in any location outside the family's home.
• Specialized Respite Support - provides families or guardians with children or youth who have exceptional social, emotional and behavioural needs with opportunities to plan periodic breaks from care giving.
• Autism Spectrum Disorder (ASD) Respite Support - offers a flexible mechanism to families or guardians caring for children and youth who have a diagnosis within the Autism Spectrum Disorder, to receive temporary relief from the physical and emotional demands involved with caring for children diagnosed with ASD.

A group of volunteers who collect and donate baskets filled with essential and special items to provide assistance and encouragement to women as they leave local shelters. Baskets delivered to Optimism Place and Emily Murphy Centre.

• 24/7 telephone hotline through Westover Treatment Centre for anyone needing help with their own or someone else's addiction

A joint program initiative between Chatham-Kent Health Alliance Mental Health and Addictions Program and the Westover Treatment Centre in Thamesville.

The A.L.L. for Kids Program helps with the cost of registering children into recreational activities in Chatham-Kent. The A.L.L. stands for Activities, Lessons, and Leisure.

A children's mental health service that links individuals and families with appropriate local resources to assist children and youth who may be experiencing emotional or behavioural difficulties. Assistance can be particularly useful if the complex needs of an individual require a response from multiple service providers. In that situation, KIDS Team can help to coordinate access to the necessary supports.

In fulfilling its role, KIDS Team staff work collaboratively with other service providers to develop and monitor a comprehensive community service plan that has been designed to meet the identified needs of the child or youth. Also, if there is a need to augment existing ministry-funded services with additional supports, KIDS Team has access to a limited amount of provincial funding that can be utilized to purchase these resources.

2nd Chance Ministry is a Christian outreach ministry to those in our community daily, who are in need of immediate, practical help and/or spiritual assistance.

Assistance may include:

• food vouchers, diapers, clothes, shoes, toiletries, etc.

• help with dentures, eyeglasses and transportation
• spiritual and emotional support
• Advocacy for help within our community
• Mentoring program is part of the Ministry.

Incontinence Supplies Grant Program - For children and youth 3-18 years of age with chronic disabilities (physical or developmental) that result in irreversible incontinence or retention problems lasting longer than six months requiring the use of incontinence supplies * the program is fully funded by the Ministry of Health and Long-Term Care

Camping - Summer camp programs for children and families at two accessible camps across Ontario * Camp Merrywood (Perth) and Camp Woodeden (London)

Huron Park residents can purchase discount transportation card for four, on-way rides between Huron Park and Exeter to access essential and non-essential services. On Demand service provided 24/7.

When you register with the Christmas Bureau, you will be scheduled a private and confidential appointment at your closest bureau site. There are five individual bureaus located throughout Huron County. To be eligible for support with the Huron County Christmas Bureau program you must have custody of your child(ren) and live in Huron County.

Skills Upgrading program helps upgrade skills for employment, apprenticeship or college, get ready for high school credits, and learn new skills to improve well-being. In the program, participants will work on lessons at their own pace, with the help of a Literacy Practitioner. It is the policy of the Foundation that all active patients of the Windsor Regional Cancer Program have appropriate access to resources to assist them with financial needs that result from the diagnosis or treatment of cancer.

• In consultation with their Social Worker, a financial assessment will be completed to determine need.
• If patients fail to qualify for a variety of community and government resources, an application to the Patient Assistance Fund can be made.
• It is intended to assist with food, shelter and medical related expenses, and other items as deemed necessary and appropriate on an individual basis.
