When able, Neighbourlink volunteers can offer clients help with food, used beds and bedding, linking used furniture donors with callers in need.

Provides customized home care services. Services like companionship, meal preparation, light housekeeping, helping with running errands, transportation, personal care, Alzheimer's care, and much more.

Provides a range of in-home services that allow seniors to live at home, safely and confidently. Offer laundry and housekeeping, in-home care, companionship, door-to-door transportation, and referrals to home maintenance chores such as yard work and snow removal, and personal safety services.

Provides eligible seniors, who due to lack of resources or appropriate supports, require assistance in returning home after a hospital stay or ER visit. The goal is to support a client's independence and reduce the risk or re-hospitalization by providing transitional support services in the home. Clients can qualify for one visit per week for up to six weeks after discharge offering free services such as transportation home from hospital, frozen meals and meal prep, essential laundry, light housekeeping and grocery shopping or pharmaceutical delivery.

Provides transportation for the frail, elderly and disabled in the community. The service is for those unable to use public transportation or private means. In town; trips to medical appointments, day programs, groceries and social outings. Out of town transportation to medical appoints only with destinations along the London-Windsor corridor.

COVID-19 - Essential services are running for transportation. Safety protocol is in place. Social transportation is provided based on availability. Volunteer driving is suspended until further notice. Subsidy is available for those who qualify.

A Christian ministry supported by 30 local churches in Chatham-Kent whose volunteers have been screened and are ready to assist in the community:

COVID-19: Face coverings are mandatory on public transit vehicles and physical distancing should be practiced wherever possible, even at bus stops. Practice good personal hygiene, including frequent hand washing with soap and water or use hand sanitizer.

A curb-to-curb shared ride accessible public transit service for persons unable to use Ride CK's conventional system due to a disability. Specialized Transit is available on an unconditional (permanent), temporary, or conditional basis, depending on the registered user's eligibility. Registered passengers may use the service for medical appointments, school, shopping, visiting family or friends and social outings within service boundaries.

Service is available within the communities of Chatham and Wallaceburg.

Transportation service that focuses on connecting people to their community by providing an affordable, accessible transportation option to rural and/or low-income people. The program has one accessible vehicle that can hold 2 passengers using wheelchairs and 7 walk on passengers.

Eligible residents can use the transportation service for a variety of needs ranging from drives to appointments and social activities or to be linked to pre-existing transportation services within Chatham-Kent such as the bus or VIA Rail.

Drives will be provided to multiple passengers at the same time, based on destination. Transportation service is offered on a first come, first serve basis.

Passengers must provide the Transportation Office with at least 3 business days (72 hours) notice to book a drive, whenever possible. Short-notice requests may not be fulfilled, based on passenger capacity.

Helps older adults and persons with disabilities maintain their independence and quality of life.

Home Help

• a broker service, which matches clients to skilled, self-employed Home Helpers, based on client-identified needs and goals.
• free in-home assessment ensures all client needs are addressed.all assessments are completed by the Home Help Service Coordinator who is a registered healthcare professional.
• affordable rates range from $20-$25 per hour for a variety of services from general housekeeping to personal support and respite.

Frozen Meals

• nutritionally balanced, home-style meals developed by dietitians.
• lactose free, gluten free, minced, and pureed meals are available by request.
• pricing ranges from $2.75 to $7 for single items; savings bundles available.
• free delivery in Chatham-Kent twice monthly.

Client Intervention and Assistance

• provides short-term social work services that support and empower individuals to live independently, with dignity, in the community.
• possible services include 1:1 supports, personal assessments, service coordination, system navigation, and advocacy.
• client directed, confidential, and delivered at no cost.

CareLink Health Transit

• provides accessible, non-urgent medical transportation services.
• trips range from local to out-of-town and may include medical or therapeutic appointments, social activities and other destinations that support the activities of daily living.
• flat rate fares vary based on single destination and 3 hour service window applies.
• additional destinations can be added at a fee of $5 per destination.
• additional fees will be applied to driver wait times in excess of three hours at $20/hour.
