Core programs include:

• Inpatient Psychiatric Services: 21-bed inpatient psychiatric unit. Focus of an inpatient admission is to treat acute symptoms in order to stabilize a patient's mental illness and initiate a treatment plan. The goal is for the patient to then successfully transition into the community for ongoing treatment and support.
• Psychiatric Assessment Nurse (PAN): A specialized mental health nurse provides an assessment to inpatients. Once the assessment is complete, a treatment decision is made either to admit or to discharge for community follow-up. PAN partners with many other community agencies with a goal of creating a treatment plan that allows the client to return to the community with a coordinated treatment plan in place.
• Community based services: Partnered with Canadian Mental Health Association Lambton Kent (CMHA LK) to provide an integrated mental health and addictions service.
• Outpatient Services: Mental Health Counseling and Treatment, Outpatient Psychiatry Clinic, Early Psychosis Intervention Program - Today Not Tomorrow (TNT), Psychogeriatric Mental Health Outreach Team (GMHOT), Addictions Assessment, Treatment and Referral Program, Concurrent Disorders Program, Problem Gambling and Eating Disorders and Assertive Community Treatment.

Mental health program for seniors:

• comprehensive psychogeriatric assessment, psychoeducation, counselling and psychiatric treatment
• works to improve the mental health of older adults experiencing symptoms of mental health
• reduces the need for psychiatric admission and visits to hospital emergency departments
• enhance the knowledge and skills of family caregivers and caregiving staff of facilities

Specialized behavioural support for children, adolescents, adults, and seniors who are experiencing behavioural, social, emotional, and psychiatric difficulties

Family Services - Provides one-on-one support for families * support focuses on addressing and stabilizing challenging behaviours * no waitlist * services provided wherever client is, including home, hospital, school or in any other community setting

Specialized Behavioural Support Services - Customized support provided in a variety of settings, including residential facilities, day programs, hospitals, schools, long term care facilities, foster homes, and community programs * flexible, client-centered services reviewed on a regular basis to ensure quality care * supports to youth justice programs for youth aged 12 to 17 who are in conflict with the law, to assist with community integration and support

Specialized psychiatric care and support for older adults who suffer from late onset mental illnesses or severe behavioural responses * program focuses on assessment, treatment, rehabilitation, prevention, family and community support and education *

Geriatric Day Program - Offers various professional programs and services including cognitive neuropsychiatry, mood disorders and psychotic disorders clinics, as well as limited therapeutic programs and therapy for outpatients

Discharge Liaison Team - Facilitates the transition of current inpatients or registered outpatients undergoing transition from hospital to the community

Regional Psychogeriatric Program - Provides clinical assessments in the patient's home, community clinic or long-term care facility, for older adults with behavioural or mental health issues

All patients see a psychiatrist as part of the initial evaluation * some inpatient consultation and education to doctors, health care professionals and long-term care facility staff

Specialized care of older persons * evaluation and treating of functional, medical, and psycho-social aspects of illness and disability

Consultation, education, community development and evaluation of services * linked to older adults evaluation teams in ten counties by the Southwestern Ontario Geriatric Assessment Network

Geriatric Ambulatory Clinics - Consultative outpatient services and specialty clinics

Geriatric Rehabilitation Day Hospital - Hospital-based outpatient day program for frail older adults * offers nursing, physiotherapy, occupational therapy, and social work services to help patients be healthy, independent, and maintain their quality of life * program collaborates with community services to ensure the right supports are in place for frail seniors at home

Geriatric Rehabilitation Unit - 30-bed hospital inpatient rehabilitation program for older adults with multiple, complex health problems * goal is to promote patient's health, functional independence, and quality of life * services provided by doctors, nurse practitioner, nurses, physiotherapists, occupational therapists, physiotherapy/occupational therapy assistants, therapeutic recreation specialists, speech language pathologists, audiologists, dietitians, and social workers

Inpatient Unit - Inpatient clinical consultation, assessment, treatment, education, research and evaluation

Outreach - Peer support groups, education, and training for family members, service providers, and caregivers * sessions may include information such as advanced directives and safety issues

Physical Maintenance Program - Half-day program aimed at maintaining the client's physical function and mobility conducted by a rehabilitation trainer and a therapeutic recreation specialist with the assistance of volunteers

Regional Geriatric Program - Assessment of frail older adults in homes, hospitals, and long-term care homes * outreach team provides recommendations to care providers, families, and patients

This regional service supports seniors who have complex medical and/or mental health needs. A team member follows up with the senior in their home once a comprehensive geriatric assessment has been completed and it has been agreed to by the senior and the care team that this service can support the individual's plan of care. The team member works closely with the senior's support network and promotes independence, self-directed care, and quality of life through the coordination of appropriate services and support for the senior.

An assessment and treatment program for seniors living in the community who are experiencing mental health concerns which may include: depression, mood changes, dementia and/or anxiety. The service is focused on the individual and their family. The team provides a comprehensive geriatric assessment that will include a medical and mental health history and screening. The care plan is developed and implemented with the individual and their support network including their primary care physician.

Addiction Program - assessment and outpatient counselling for people of all ages experiencing issues with alcohol, drug use and gambling. Mental health and peer support also available. Assistance is available to family and friends. Additionally, counsellors are available through the guidance offices of high schools. Community education is a major component of this program

Adult Mental Health Program - community-based, client-focused services to adults with mental illnesses or with concurrent disorders (mental illness and addictions) including assessment, consultation, treatment, education, support, and advocacy. It is an interdisciplinary staff (physicians, nurses, social workers, with support from addiction counsellors and peer support as required)

Specialized Geriatric Services - provides non-emergency clinical assessment, consultation, treatment and education to older adults, their families, and service providers who are, or know of, someone who is experiencing mental health issues combined with age-related difficulties

Community based mental health services for individuals and families facing mental illness and developmental challenges. Services include:

• specialized psychiatric and mental health assessments and treatment for adults and seniors; treatment includes brief individual and group therapies, education and support groups, prescribing and monitoring of medication
• programs for school-aged children and teens
• specialized programs for Eating Disorders

Intensive Geriatric Service Workers (IGSW) - work with frail seniors after release from hospital or who display a complex number of needs to create a care plan for community supports and health care

Family Support Options - program for families caring for children with developmental or physical disabilities; can help families apply for funding to receive one-to-one support, respite services, access to Supporting Kids in Camp program; Infant and Pre-School Support Services including pre-school assessment consultation for child care providers and families of children 2-6 years of age; call 1-800-265-7293 ext 3616 for details

Court Support Worker - support for people with mental health issues who are in conflict with the law

Counsellors are skilled in Brief Solution Focused Therapy and Crisis Intervention to provide the following:

• Crisis Outreach and Support Team (COAST) referrals
• connection to addiction services
• safety and crisis planning
• coping skills and self-soothing management
• referrals and connections to on-going service as required
• Naloxone kits and harm reduction supplies

Regional service to assist family, community and long-term care providers in the continuing care of older adults with complex cognitive, emotional and behavioural disorders

• specialized assessment, consultation, treatment and short-term specialty case management and intervention

• Mobile Crisis Rapid Response Team (MCRRT) - program partners a uniformed OPP officer with an experienced mental health professional to respond as first responders to calls through 911
