Assertive Community Treatment (ACT) provides treatment, rehabilitation and support services for individuals with severe and persistent mental illness in Sarnia-Lambton who require intensive service to live independently in the community.

Individualized services that address mental and physical health, addiction, education, vocation, social development/functioning, activities of daily living and family, and other relationships, are provided to the client in their homes and the community.

Using an integrated approach by a multidisciplinary team of mental health professionals, ACT works with clients to reach their individual goals and lead healthy lives.

Dedicated to helping children and youth in coping and adapting to life's challenges, and promoting the well being of infants, children, and youth who are experiencing mental health related difficulties. Work with parents and the community to provide integrated early intervention, prevention, and non-residential treatment services. All supports provided free of charge to children and youth from birth through age 18 and their families.

Supports the well being of individuals and families through the coordination of professional counseling services, within or outside the community, art and play therapies targeting children and youth, case management services for individuals seeking mental health and/or addictions services in addition to addictions counseling and referral to treatment centres, medical detoxification, medical care and/or methadone programs.

Addiction Transitional Case Management Service - Assessment and Referral - Program facilitates discharge process from London Health Sciences Centre and the Stabilization Space * ensures patients have linkages to appropriate addiction and mental health services and supports upon discharge * patient works with Addiction Transitional Case Manager to create and implement plan for care

Addictions Supportive Housing - Offers housing opportunities as a part of addiction treatment planning

Back On Track Program - Provincially mandated program for convicted impaired drivers and drivers who receive two or more provincial alcohol or drug related driving offences

Community Opioid Addiction Program - Offers direct client services and community education for people with opioid addictions * assessment and screening required for entry into the treatment program

Community Withdrawal Support Program - Flexible program helps people with problems related to the use of alcohol, drugs (legal and/or illegal) supports people before, during and after periods of intoxication and/or the process of withdrawal * services provided by a team of registered nurses, delivering services over the phone, face-to-face and via Ontario Telemedicine Network (OTN) technology

French Mental Health and Addiction System Navigation Program - Case Management - Provides access to mental health and/or addiction support services in French

Heartspace Program - Specialized prevention, early identification, education and treatment program for women who are pregnant, parenting and substance-involved and their children (prenatal to six years of age)

Problem Gambling and Behavioural Addictions Program - Treatment planning and community-based outpatient treatment for people with gambling, gaming, internet/technology, shopping/overspending or pornography/sex concerns

Rapid Access Addiction Medicine (RAAM) Clinic - Community-based, low barrier services for clients seeking treatment for any substance use disorder * focuses on addiction medicine and offers both medication and psychosocial interventions * community education about addiction medicine for health care professionals

Smoking Treatment for Ontario Patients (STOP) Program - Smoking cessation treatment and counselling support

Substance Use Services - Treatment planning and community-based outpatient treatment for people with a substance use concern

A children's mental health service that links individuals and families with appropriate local resources to assist children and youth who may be experiencing emotional or behavioural difficulties. Assistance can be particularly useful if the complex needs of an individual require a response from multiple service providers. In that situation, KIDS Team can help to coordinate access to the necessary supports.

In fulfilling its role, KIDS Team staff work collaboratively with other service providers to develop and monitor a comprehensive community service plan that has been designed to meet the identified needs of the child or youth. Also, if there is a need to augment existing ministry-funded services with additional supports, KIDS Team has access to a limited amount of provincial funding that can be utilized to purchase these resources.

A not-for-profit organization run for and by persons living with mental illness. Daily programs are offered including peer support, education, social-recreational opportunities, advocacy, networking and community resource navigation. All persons living with mental illness are eligible to participate in programs.

Supports to people with a serious mental health illness or a dual diagnosis living in 8 unit apartment building located in a rural setting

Supports include but are not limited to stabilization and recovery, life skills, medication and health, recreation, community participation, healthy relationships and employment * usually roommates within an apartment are the same gender

3 unit co-ed group home providing housing for 4 people per unit with a dual diagnosis and/or a serious mental illness

Daily 24 hour support including stabilization of mental health, life skills, medication and health monitoring, recreation, community participation, healthy relationships, employment * participants must be willing and able to live cooperatively

Support and services to people with a serious mental health illness

Participants reside in various settings in the community, including group home, apartments and family homes * supports are offered in all aspects of life including recovery, life skills, medication administration, health monitoring, recreation, community participation, healthy relationships and recreation and leisure. This program is intended for persons with greater independant skills.

Supports adults with a serious mental illness and/or a developmental disability by providing services that foster personal growth and participation as full citizens in their communities

Huron Community Mental Health Services offer skill based educational groups, therapy groups, psychosocial rehabilitation groups, time limited individual counselling based on severity of illness and advocacy.

28 bed (23 HSC (Homes for Special Care) and 5 private) adult assisted living facility for people with a mental health diagnosis. Services include housekeeping, assistance with daily living activities, cooking, laundry and medical transportation.

CMHA Thames Valley Addiction and Mental Health Services - Exeter and Goderich provide a range of mental health supports to individuals living in Exeter and southern Huron County including life skills education, linking to resources, advocacy, individual goal setting, personal growth and recovery. The following programs and services are provided at the Exeter location:

• Permanent Supportive Housing (Exeter): Four tenants in safe affordable housing where rent is geared to income. Tenants receive on going support as they gain greater independence
• Supportive Housing: individuals who are homeless or at risk of becoming homeless in Goderich and Exeter. Rent-subsidized apartments provide independent living for tenants. Individuals are supported as they re-integrate into community
• Case Management: staff help individuals develop skills, increase self sufficiency and become more connected with their community CMHA-TVAMS offers community supports with varying levels of intensity and participates in shared care arrangements with other community support provided
• Community Wellness: Provides opportunities for individuals to enhance their life skills through leisure and community engagement. Helps to increase a sense of personal wellness and quality of life

Support for multiple myeloma patients, their caregivers and family members.

Provides comprehensive diagnostic assessment and consultation for children and youth with complex symptoms that suggest a mood and/or anxiety disorder.

The Child and Parent Resource Institute (CPRI) team consults with community partners and develops a treatment and safety plan for your child.

CPRI does not do risk assessments, emergency or crisis intervention or parent or custody assessments.

For children and youth who display concerning sexualized behaviour which has a negative impact on their relationships at home, school and in the community.

The Sexual Behaviour Team (SBT) goal is to understand your child's problem sexual behaviour and find ways to make sure that they and others are safe. SBT will talk to you, your child and your community supports to understand the reasons for the behaviour.

SBT will consult with your child or youth, caregivers and community partners to come up with a treatment and safety plan. We will also try to connect you to services in your community so your child can get the help needed closer to home. SBT will provide healthy sexuality resources and education presentations for community supports like group homes, schools, and children's mental health agencies.
