A retirement residence offering both independent and assisted living.

Independent Living - for adults who can live independently, but may also require some assistance. In addition to room and board, activities, entertainment, access to grounds/gardens, this basic level of care includes:

• Staff available 24 hours a day
• 24-hour emergency response
• Medication monitoring
• Weekly personal laundry and linen cleaning
• Light housekeeping including weekly trash removal

Assisted Living - for residents who are able to direct their day but require skilled or extra services. For an additional monthly charge, Assisted Living includes all the elements of Independent Living with the addition of the following care services plus up to maximum 60 minutes additional support in a 24-hour period:

• Daily light housekeeping and trash removal
• Medication administration
• Bathing assistance as needed
• Personal daily laundry assistance, or as needed
• Continence management and personal hygiene assistance as required
• Personal assistance with feeding, dressing, AM/PM personal care daily or as required
• Assistance with ambulation as required

Assisted Living Plus - for an additional monthly charge, includes the same care support as Assisted Living, but also includes up to 90 minutes support in a 24-hour period.

Respite & Convalescent Care - two short-stay programs intended to support individuals who may benefit from or require a short-stay in an Assisted Living home for recuperation. Offering a selection of short term stay suites. The rooms are equipped with all the services and amenities available in any of three lifestyle options, and can be customized to meet individual needs.

A social service agency that provides supports for adults with Autism and Developmental Disabilities.

Personal care for frail older adults who have difficulty caring for themselves but who are still capable of living independently in their own apartment

Residential supports for people who experience developmental disabilities.

Supported Independent Living - This model of service is an option for a person who chooses to live independently or with a roommate. The level of support is determined on an individual basis and can range from several hours per day to a few hours each week.

A privately owned retirement and assisted living home providing assistance with activities of daily living (medication administration, assistance with activities of daily living, 3 nutritious meals/day and snacks, laundry, light house keeping, activities and wellness programs and more). Private rooms available with 2-piece bath.

Currently have 3 supportive housing programs, with a total of 18 fully accessible and 11 partially accessible apartments with round-the-clock non-medical care available to assist tenants with all Activities of Daily Living including but not limited to, personal care routines, hygiene, feeding, transfers, medication administration and meal preparation.

Provision of on-site Direct Support Professionals in housing situations up to 24 hours per day, 7 days per week at various locations in Chatham-Kent. Supports are based on people's abilities and wishes.

Provides and advocates for a full range of community-based services for people who have intellectual disabilities including accommodation, employment, respite, community access, planning and family support.

Community Living provides services and supports to people with developmental disabilities and their families with a focus on community integration as well as participation in all aspects of community life. Work experience and training are provided through a local company. Volunteer work placements are set up throughout Huron County. Social and recreational activity opportunities are made available for residents. Supported by Arc Angels Thrift Shop located next door.

Supports to people with a serious mental health illness or a dual diagnosis living in 8 unit apartment building located in a rural setting

Supports include but are not limited to stabilization and recovery, life skills, medication and health, recreation, community participation, healthy relationships and employment * usually roommates within an apartment are the same gender

3 unit co-ed group home providing housing for 4 people per unit with a dual diagnosis and/or a serious mental illness

Daily 24 hour support including stabilization of mental health, life skills, medication and health monitoring, recreation, community participation, healthy relationships, employment * participants must be willing and able to live cooperatively

Support and services to people with a serious mental health illness

Participants reside in various settings in the community, including group home, apartments and family homes * supports are offered in all aspects of life including recovery, life skills, medication administration, health monitoring, recreation, community participation, healthy relationships and recreation and leisure. This program is intended for persons with greater independant skills.
