Partners with the Victorian Order of Nurses to provide a health care clinic for children, adults, and families who do not have a family physician or nurse practitioner

Services include:

• Annual health examination
• Assessment, diagnosis and treatment of health conditions
• Health promotion

• Referrals to specialists and other agencies
• Sexual health services

Offers primary health care services including but not limited to:

• Chronic disease education and prevention
• Disease management

Provides services that promote physical and mental health for community members

• Provides health awareness and counselling, chronic disease management, education and prevention, especially in the areas of diabetes and alcohol/drug abuse
• Coordinates clinics/workshops in areas that may include Blood Pressure, Environmental Health, Immunization, Sexual Health and Communicable Diseases

Addresses individual health care needs by providing:

• Advocacy on behalf of individual with other health professionals
• Coordination of pre-natal care and classes
• Home visits to the sick, elderly, new mothers and persons with health care needs
• Delivery of various health promotional programs such as the Annual Health Fair, Health Career Fair, Active Seniors and Diner's Club
• Assistance with coordinating local and long distance travel arrangements for medical appointments

• Provides programs and services that promote physical and mental health for community members
• Offers a health centre consisting of a multi-disciplinary team that provides health prevention, promotion and non-urgent treatment
• Focuses on a holistic approach offering both western and traditional healing choices
• Provides health awareness, skills development, education and prevention workshops and seminars
• Coordinates clinics in areas including Foot Care, Blood Pressure, Immunization, Sexual Health and Communicable Diseases
• Offers diabetes education and management counselling
• Provides mental health counselling

COVID-19 Response
During the COVID-19 Pandemic, the office will be closed to the public until further notice. Appointments will be conducted virtually (by phone or video). All group programs are temporarily on hold.

Regular Service
Medical care team focused on family and community health. Coordinates patient care in order to assess and treat injuries or illness. Team includes family physicians, nurse practitioners, registered nurses, mental health therapists, respiratory therapist and dietitian.

Provides quality end-of-life care to patients and families with an advanced progressive illness

Geriatric Care:

• Provides a comprehensive assessment and assists seniors with complex health problems.
• family and caregiver support related to senior's care including: depression, dementia, confusion, memory, falls, medication problems
• has a geriatric outreach team to provide care in home

Pulmonary Rehab/Asthma/Smoking Cessation:
Offers self-management programs for people and their families who live with asthma or COPD (Chronic Obstructive Pulmonary Disease), as well as smoking cessation.

Dietitian services:
Offers assistance to patients with:

• Weight management
• Elevated cholesterol
• Diabetes prevention
• Irritable bowel disease and other digestive disorders
• Childhood nutrition
• Sport nutrition
• Healthy eating on a budget

Mental Health Therapy services:
Counselling program offers short term counselling to adults, children, and youth for issues such as:

• Anxiety
• Depression
• Grief
• Couple issues
• Stress

Primary Care services:
Registered Nurses and Registered Practical Nurses offer chronic disease management and health promotion programs in the doctor's offices of the Algonquin Family Health Team.

The Muskoka Community Health Hub - Dorset:
Provides primary and episodic care to local and seasonal residents. Not a walk-in clinic. Located at 1096 Main St, Dorset, ON P0A 1E0 Phone: 705-766-0866 for more information.

A collaborative effort with community partners to provide visible, accessible, and reliable health promotion and healthy living information, education, and programs to the community.

Programs and services include:

Healthy Heart Program:
Provides educational sessions to assist those with or at risk for heart disease to improve their lifestyle and prevent future illness. Eight week exercise component that provides cardio and resistance training.

Nurse Practitioner (NP):
The NP provides general primary health care services for patients of all ages, and are involved in a variety of programs

Diabetes Prevention Program:
A free program for adults who are at high risk of developing Type 2 diabetes, with the following risk factors: overweight, especially in the stomach area; high blood pressure; high cholesterol; low physical activity levels; Diabetes during pregnancy.

Breastfeeding Support Clinic:
Free Breastfeeding Support Clinic with lactation consultant held once a week.

Regular Service
Nurse Practitioner led clinic. Provides primary care to rostered people without another healthcare provider, including health assessments, prescriptions, laboratory or diagnostic tests, wellness care, chronic disease management and referrals. People with another healthcare provider, or people who are not rostered, can call to request care for episodic concerns, which include short-term mild illnesses and injuries. Services including injections/vaccinations, blood work and sutures are also provided.

Acts as a walk-in clinic during the summer on Sat, Sun and holiday Mon.

Operates as a health centre focused on family and community health for the Francophone population in Sudbury
