Helps individuals with developmental disabilities live to their highest level of potential

• Connects individuals with an Adult Protective Services Worker and Family Support Worker
• Assists with the completion of Ontario Disability Support Program (ODSP) applications
• Mediates between tenants and landlords

Case Management

• Assesses, plans, follows up and coordinates care
• Monitors individual situations and helps individuals select the best option of all the services and supports available

Advocacy

• Ensures (within the scope of authority) the rights of the individual with the disability are acknowledged
• Informs individuals of their rights
• Works within the areas of housing, education, health, and the law

Person Centered Planning (PCP)

• Develops a plan tailored to the individual to promote their choices, services and supports
• Identifies an individual's personal, educational, vocational, medical, and safety support needs in relation to wants
• Connects and supports individuals in accessing resources to meet identified needs and wantsl

• Provides clinical support services to children and adults with an intellectual and/or developmental disability
• Services can be delivered on an individual basis, group sessions or through a mediator
• Promotes the social and emotional well-being of the individual

Offers the following services:

• Behavioural

• Speech and language pathology (adults only)

• Video conferencing site

Operates a full service restaurant/cafeteria and catering business that provides employment and a teaching environment for individuals with an intellectual disabilities in the areas of:

• Basic food preparation and baking
• Cashier training
• Communication skills
• Customer service
• Setting and clearing tables
• Teamwork
• Time management
• Transferable skills

• Facilitates transition from school to community life through a self-directed Transition Plan
• Coordinates plans to support personal goals
• Provides opportunities to build connections within the community
• Assists with applications for funding and referrals to community services

Students receive support to pursue individual goals including:

• Education
• Community connections
• Vocational opportunities
• Independent living skills
• Rights and advocacy

• Provides services and supports to meet the needs of adults with an intellectual and/or developmental disability
• Works on a person-centered model to address the values, priorities and preferences of each individual

• Provides support services to individuals residing in the community
• Offers varying amounts of support and is based on an individual's needs
• Supports required are determined by the individual receiving supports and their families

• Money handling and budgeting

• Empowers individuals with intellectual disabilities to become physically fit, productive and respected members of society through sports training and competition
• Offers children and adult athletes year-round training and competition in many Olympic-type sports at Invitational and Regional meets

Offers opportunities for volunteering within all sports offered

Single point of access for children and youth to be placed on the Ontario Autism Program (OAP) waitlist for autism services

• Facilitates an integrated and coordinated intake process to identify needs of children and youth with Autism Spectrum Disorder (ASD) and their families
• Provides access to Family Support Workers to assist families with service and program navigation and support, including direct funding option

Regular Service
Provides comprehensive client-centered, professional and skilled services to infants, children, youth and families, and adults with a developmental disability. Addresses social, emotional, behavioral, physical and developmental difficulties. Improves the well-being of individuals, families and the community through clinically effective partnerships.
