Community-based support within housing for people with serious mental illness and addictions who are living within housing formerly managed by CMHA - Nipissing Regional Branch

Program includes:

• Education
• Conflict resolution
• Crisis prevention
• Housing assessment and follow-up
• Referral to community resources

Provides community-based support within housing for individuals with serious mental illness and addictions who are living within housing formerly managed by CMHA - Nipissing Regional Branch

Program includes:

• Conflict resolution
• Coordination of required services for tenants, family members and landlords
• Crisis prevention
• Discharge planning
• Education for tenants
• Housing supports
• Referral to community resources

• Provides personal support workers to assist individuals with personal care, housekeeping, and laundry
• Helps individuals remain independent in their own homes for as long as possible

Assistance is provided for the following:

Personal Care

• Help with bathing, transferring, grooming, medication assistance, and other activities of daily living

Meal Assistance

• Helps with heating and serving a meal that was prepared in advance

Housekeeping

• Helps with vacuuming, cleaning bathrooms and kitchens, and mopping floors
• Scheduled according to a service plan
• Assistance with laundry

Provides a variety of non-medically based support services to assist people with physical disabilities, including:

• Attendant Care Supportive Housing
• Attendant Outreach
• Enhanced Congregate Care
• Loan Out Cupboard
• Post-Stroke Rehabilitation Program
• Transition to Home Program

• Provides support services to individuals residing in the community
• Offers varying amounts of support and is based on an individual's needs
• Supports required are determined by the individual receiving supports and their families

Types of support offered include:

• Accessing community resources
• Assistance with appointments
• Home maintenance
• Landlord and tenant negotiations
• Money handling and budgeting
• Meal preparation assistance
• Shopping

• Functions as a voluntary service in which an owner/operator of a home provides residential care to individuals with a serious mental illness
• Allows individuals to live independently in the community with some assistance

Services include:

• Meals, laundry, medication management and some level of 24-hour supervision on a short-term or long-term basis
• Funding for personal care items, such as toiletries, clothing, eyeglasses, dental care, medical transportation, social and recreational activities
• Support from staff of the home

Provides individuals with an intellectual disability with the opportunity to live with a family in the community

• Operates two residential group homes for individuals with a developmental disability who cannot live alone
• Provides a range of services to support independent living based on the individual's support plan which outlines the person's interests and goals and are developed with the person and/or their family
• Enriches the quality of life by encouraging people to maintain their independence and learn new skills

Programs include residential and community participation supports:

• Two group homes in Powassan.
• Enhanced Support Independent Living Program in Sundridge
• Supported Independent Living Program provides regular support to individuals living on their own in the community.
• Community Work Experience provides employment supports and guidance including job training, job placement and assessment. Individuals supported by Almaguin Highlands may be hired to do basic property maintenance such as grass cutting, raking and simple gardening, or snow shoveling
• Community Participation Program- helps individuals access recreational and leisure opportunities within their communities. Also provides assistance in the areas of personal management, social development, household management, basic academic skills, service and community awareness
• Passport Program- Administration of Passport Funds, taking care of all paperwork and recouping directly from the Passport program AHCL can aid in navigating these funds in two ways: Either by brokering services from outside qualified contract workers or by directly providing services through internal resource. This program helps adults access recreational activities within their communities and provides additional respite to care givers.
• Family Support Services Program provides assistance to families with a child under age 18 who has intellectual disabilities. Information on developmental delays, community resources, support counselling, government programs. Referral to appropriate resources. Assistance with applications and advocacy. Financial assistance
• Respite Program for adults and children
• Family Home for adults
• Transitions assists youth and young adults with an intellectual disability in transitioning from school to life and as adults in the community

Provides a retirement/boarding home environment for up to 6 individuals as an alternative to a city retirement or institutionalized living

Services include:

• Home cooked meals
• Laundry services
• Housekeeping
• Transportation for appointments
• Social outings

Offers children, families and adults, including transitional-aged youth, who have a developmental disability the supports they require to be engaged in their community as fully as they choose.

Supports are individualized, and based on interests and abilities, and may include:

• Skill building
• Leisure activities
• Learning, including options for post secondary education
• Social activities
• Volunteering
• Work, and other activities directed by individual choice

Provide services and supports to people with developmental / intellectual disabilities and their families. Focus on community inclusion in all aspects of community life.

Children's Services provide support for families to obtain services and advocate for their children including:

• Resource Teacher Program for families with children ages 2 to 6
• Family Support Program for families with children ages 6 to 18

Adult Services include:

• employment skills

Adult residential support is offered in several settings through people's own homes, group homes and the Supported Independent Living Program.
