HEALTHY HEART SCHOOLS' PROGRAM: Primary prevention program provides grade nine students with information about their individual heart health profiles to guide them toward making responsible decisions (in partnership with Niagara Region Public Health, the various local school boards, Brock University, and Sick Kids History Childhood Obesity Research Team). Within a five day teaching block, the program provides measurements of fitness, height, weight, blood pressure, total cholesterol and CPR training. Provides the opportunity to present information on reducing the risk factors of heart disease to every person in the Niagara Region at least once in their lives. CORPORATE WELLNESS PROGRAMS: Corporate health services provide employers with the education and tools to create a healthy and energetic work culture.

PUBLIC ACCESS DEFIBRILLATION (PAD) PROGRAM: A team with background in pre-hospital care provides quality assurance in maintaining PAD programs. Provides defibrillation training and advocates for timely access to defibrillation throughout Niagara. For information call 905-358-5552 ext 102.

HEALTHY DAILY LIVING PROGRAM (HDL):

• Prevent and reduce future risk of heart disease
• Manage diabetes better
• Reduce risk of more heart problems for people who have had angina, a heart attack, by-pass surgery, heart failure or a history of heart disease
• Improve good cholesterol
• Change lifestyle choices
• Offers a variety of workshops as well as risk reduction services such as: clinical assessment for risk of heart disease, intervention to help quit smoking, lower blood pressure, improve cholesterol and blood sugars
• Fitness: reviewing personal risk score and fitness to choose an activity plan based on one's cardiovascular risk level (fee charged)

COOKING FOR A HEALTHIER YOU: Provides cooking classes at a location determined by the group/organization/corporation. The cost will cover tastings of food prepared and hand outs. For information call 905-358-5552 ext 118.

HeartCORE: Furnished with MedX circuit training equipment. Weekly circuit style workouts are completed in less than 15 minutes and are performed with slow, controlled movements. An energy coach provides one-on-one attention, ensuring proper technique and safe, effective progression of the exercises.The program is suitable for most people as it offers individualized instruction, resistance loads that are geared toward age and initial fitness level, plus the weekly sessions encourage adequate recovery time. Weekly exercise sessions will increase muscle strength, flexibility, bone health and much more. Additional health benefits include better insulin sensitivity and lower cholesterol. Partnerships within the Heart Niagara office inclide: Epilepsy Niagara, Family Counselling Centre (Tue) and Community Tax Clinics.

A program that creates opportunities for community members to learn about healthy lifestyles and participate in active living activities. Provides the urban Aboriginal community with hands on experience through:

• interactive workshops
• healthy nutrition information

• sport and recreational activities

Also focuses on youth leadership by encouraging and supporting youth to promote healthy lifestyles amongst their peers and to be healthy living leaders in their community. The program will prepare youth for leadership roles through training, mentoring and community event involvement.

Enforces the smoking by-laws and Smoke Free Ontario Act within the Niagara Region. Available to report offences. The hotline also is a service to smokers who want information or who want to quit. Information can also be obtained on second hand smoke issues and the law.

COVID-19: Groups are currently run virtually. Not all groups are run ongoing.

CHRONIC PAIN SELF MANAGEMENT: A 6 week workshop for people who live with an chronic or ongoing pain who want to learn ways to better live with their pain and improve their quality of life.

GENDER QUEST: A support group for adults in any stage of gender transition, including those questioning their gender, modeled after Sherbourne Health Centre's Gender Journeys group.

HOT TOPICS IN HEALTH: A monthly drop-in session that provides information on different Hot Topics in health as chosen by our clients. Topics could range from health conditions such as high blood pressure or diabetes to healthy behaviours such as sleep and mindfulness.

QUIT SMOKING GROUP: A smoking cessation program that helps people develop a personal quit plan, deal with any barriers or challenges related to quitting and access 6 months worth of free Nicotine Replacement Therapy (eg. patch, gum, lozenges and inhalers). * limited access currently to COVID-19*

TAKE CHARGE! CHRONIC DISEASE SELF MANAGEMENT: A 6 week workshop for people who live with an ongoing health problem who need help identifying coping strategies, setting goals, managing stress, making healthier lifestyle choices, working with their healthcare team and living life to the fullest.

TRANS/GENDER-QUESTIONING YOUTH GROUP: A monthly social support group for gender diverse youth aged 14-19 years. Provides an opportunity to meet other trans and gender questioning youth for discussion, movies, guest speakers and snacks.

A free and confidential harm reduction program that offers sterile injection and disposal supplies to people who inject drugs.

The following services are available:

• needles and syringes in various sizes
• sharps containers
• ties, filters, alcohol swabs, sterile spoons

• non-judgemental supportive counselling and referrals

Addiction Program - assessment and outpatient counselling for people of all ages experiencing issues with alcohol, drug use and gambling. Mental health and peer support also available. Assistance is available to family and friends. Additionally, counsellors are available through the guidance offices of high schools. Community education is a major component of this program

Adult Mental Health Program - community-based, client-focused services to adults with mental illnesses or with concurrent disorders (mental illness and addictions) including assessment, consultation, treatment, education, support, and advocacy. It is an interdisciplinary staff (physicians, nurses, social workers, with support from addiction counsellors and peer support as required)

Specialized Geriatric Services - provides non-emergency clinical assessment, consultation, treatment and education to older adults, their families, and service providers who are, or know of, someone who is experiencing mental health issues combined with age-related difficulties

Offers Opioid Substitution Therapy (ex. methadone treatment) for opioid addiction as well as Cognitive Behavioural Therapy and counselling to those addicted to street and prescription drugs.

A combined methadone clinic and Rapid Access Addiction Medicine (RAAM) clinic that provides assessment, brief counseling, and medication-assisted treatment for a variety of substance use disorders including opioids, alcohol, stimulants, cannabis, smoking cessation, and benzodiazepine tapering guidance

The Health & Hope Partnership

The Phoenix Program

• Early Intervention Service for Psychosis, a clinical partnership between Joseph Brant Hospital, Halton Healthcare Services, North Halton Mental Health Clinic, Halton ADAPT and the Schizophrenia Society of Ontario
• provides comprehensive clinical services for individuals in their first episode of psychosis
• eligible to individuals between the ages of 14 and 35 yrs who are experiencing symptoms of psychosis and who have received no previous treatment (or less than 6 months of treatment) for psychosis, and who live in the Region of Halton

Halton Geriatric Mental Health Outreach Program

• assists older individuals with mental health issues, and those who may have substance abuse concerns which require clinical services outside of an office setting

Prioritizing Health through Acute Stabilization and Transition (PHAST) Partnership - Burlington

• a partnership between Joseph Brant Hospital, Halton ADAPT, Canadian Mental Health Association (CMHA) - Halton Region Branch, Reach Out Centre for Kids (ROCK), and the Burlington Family Health Team (BFHT)
• provides rapid assessment and treatment services to patients aged 16 and older during a mental health crisis, and provides options for ongoing community based care (through ADAPT, CMHA, ROCK and the BFHT)

ADAPT offers a variety of Community Justice programs to youth and adults, providing assessment, counselling and case management services, assistance can also be provided to the family or caregivers

Extra-Judicial Programming (EJM and EJS) (ages 12-17)

• Extrajudicial Sanctions (EJS) is a program that gives young people an opportunity to accept responsibility for their actions and to make amends by completing certain tasks, after being charged with an offence
• Extrajudicial Measures (EJM) is a program that gives young people, who are willing to participate, an opportunity to accept responsibility for their actions and to make amends by completing certain tasks (option available to police instead of laying a formal charge)
• involvement with ADAPT may be one of the tasks assigned as part of the EJS/EJM contract

Youth Justice Program

• provides screenings, assessment and counselling services to young people involved with Halton Youth Justice Services who are experiencing issues related to drug and /or alcohol use
• referrals are initiated by Halton Youth Justice Services
• support is also provided to family members of the young person

Probation and Parole Program

• provides alcohol/drug use and problem gambling assessments for probation and parole clients as required by a court order and/or parole certificate
• these screenings, in conjunction with the Probation and Parole Officer, determine the level of service required to meet the counselling condition of the order
• services include group and individual counselling and referrals to community programs/services tailored to the needs of the individual

Drug Court Program

• the Halton Drug Treatment Court (HDTC) is a judicially-supervised drug treatment and rehabilitation program for offenders whose criminal conduct is motivated by drug addiction
• expected to take 9 to 12 months to complete
• participation is voluntary and includes an application and screening process by the Federal Crown Attorney's office and the Court Liaison worker
• once accepted, ADAPT is the lead provider to address the substance addiction and the individual is assigned an addiction worker

Safe Beds

• a short-term residential bed program (Mental Health & Justice Beds) providing individuals living with mental illness, and recent or current involvement with the criminal justice system, with a 30-day stay with onsite 24-hour supports to assist in resolving the current crisis and creating strategies for long-term interventions
• referrals are made by Halton Regional Police and admission to the program is based on a screening with Safe Beds program staff
• Canadian Metal Health Association (CMHA), Supportive Housing Halton (SHH) and ADAPT work in collaboration to provide individuals with a service delivery model that is comprehensive
• if the individual is in need of addiction services, the ADAPT Safe Beds Addiction Worker will be assigned to provide services

Trained counsellors provide ongoing support to clients of all ages who are struggling with any of the following:

• gambling-related issues
• gaming
• excessive internet use
• shopping
• hyper-sexuality

Support services begin with a comprehensive assessment to explore the effect the problem area has on all aspects of one's life, followed by an individualized treatment plan, developed by the counsellor and the client

• gambling addiction treatment plan may include individual, group, couples/family counselling, as well as referrals to day treatments, residential treatments, or utilization of other community resources
• counselling services are also available to the significant others and family members affected by the problem behaviour of a loved one, regardless of whether or not the loved one is a client of ADAPT services

Problem Gambling Intensive Day Treatment Program

• offers a no-cost, 5-day intensive treatment program focused on gambling-specific issues
• through education and support, strive to assist clients in building a successful foundation for healthy, long-term lifestyle changes
• program is accessible at several locations in Ontario and runs Mon to Fri, 9:30 am-3:30 pm
• topics covered include: education about problem gambling and how it can become addictive, exploring thoughts that maintain problem gambling, coping with emotions, developing behaviours that support change, and relapse prevention

Addictions Supportive Housing (ASH)

• program holds 14 subsidies in the Mississauga/Halton health region, and 17 in the Burlington health region
• a partnership between ADAPT, Summit Housing & Outreach Programs, and Support House
• program provides support and services to clients in recovery who are homeless, at risk of homelessness, or inadequately housed
• each participant receives a full package of clinical/support services, as well as a housing subsidy
• housing subsidy is active for a period of 12-18 months, and is based on need
• provides intensive outreach services, with case managers who support clients in their recovery and the move to successful, independent living
• clients must be stable enough in their recovery and their mental health status, within the program's time- frame, to be considered for independent living
• each client receives individual support, as well as participating in 15 life skills courses that assist in their growth and attaining independence
