Patients will be seen by an Advanced Practice Provider who will:
- Review medical history
- Ask questions about patients health
- Ask patient to complete some tests to see how well they can move their hip or knee and see what causes pain
- Review diagnostic results (i.e. X-Rays, MRI findings) as it pertains to joint(s) being assessed
- Discuss treatment options
- Develop a treatment plan that is specific to the patient
- Medical transportation services
- Medical escorts
- Referrals to community services and Friendship Centre programs
- Preparing forms and applications
- Congregate dining services (includes meal and activity)
- Home visits/friendly visits
- Security checks (in person or over the phone)
- Caregiver Support services (respite and training)

- Assistance with housing issues
- Transitioning from the Ontario Disability Support Program (ODSP) to Old Age Pension
