Emergency and transitional shelters, street outreach, support services and help to find permanent housing

Community-based early intervention program that is designed to help young people who are at risk of or in the early stages of homelessness. Services offered include: family mediation and reunification, school re-engagements and retention support, intentional case management, engagement in employment, and assistance identifying and connecting to both natural and formal support networks.

Multi-agency resource centre for at-risk youth residing within the community. Case coordinator assists the youth in navigating community and internal services based on their identified needs.
Services include:

The Notre Dame Community Resource Centre, is a multi-agency resource centre for at-risk youth residing within the community who are between 16 and 21 years of age. Youth have access to a number of outreach support services and programs to assist them in remaining housed; diverting them from homelessness and street involvement.

A case coordinator assists the youth in navigating community and internal services based on their identified needs.

Drop-in centre offering daily meals and recreation, harm reduction groups, health care services, professional services, housing support, and social engagement programming. Includes onsite health clinic and Ontario Works office.

Drop-In program that provides spiritual and social support and connections to community services.

A street outreach program for women in the sex trade. Offers support, friendship, assitance, and prayer.

Street Outreach Services that engage individuals who are experiencing mental illnesses and are homeless or living in shelters. City of Hamilton Mental Health and Street Outreach staff work in the community at many locations. This team helps persons who are homeless connect to income sources, health care, housing and support services. The team works on the street, in shelters and in the community.

Stepping Stone helps individuals and families experiencing homelessness step forward on a path toward securing permanent housing in Guelph-Wellington through housing focused programs and individualized support for adults ad families.Areas of focus and service include:

• preventative action to help those at risk of homelessness
• individualized plans for those struggling to attain a long-term residence

Prevention & Diversion:

• prevention assistance supports households in preserving their current housing situation
• diversion support assists adults and families in finding housing outside of shelter
• prevention and diversion services help stabilize current situations while assisting with a permanent housing solution

Housing Focused Outreach:Housing Focused Outreach worker visits locations known to have individuals who are precariously housed, at risk of homelessness, or experiencing homelessness. Services include:

• assess needs of individuals and families through a standardized assessment tool
• assist with aquiring suitable housing
• identify intervention recommendations
• supports their transition to a case manager

23 Gordon St. Overnight Emergency Shelter:

• previously known as the Warming Centre
• provides warm meal, shower, and overnight shelter
• accommodates up to 27 people ; one bed dedicated to the safe bed program, on a first-come, first-served basis
• supported by Diversion team, Housing Stability Outreach and Housing Focused Outreach

Grace Gardens Permanent Supportive Housing:

• individuals experiencing chronic homelessness, mental health issues and / or addiction
• Mon-Sun, 24/7 support services

Housing Focused 24/7 Supportive Shelter:

• supports approximately 40 people who are experiencing homelessness
• Stepping Stone works in collaboration with community organizations to deliver mental health, addiction, and medical support to individuals staying at the shelter
• focus is to provide access to food, showers, nursing, harm reduction, and court support
• individuals have their own rooms, common space, etc.
• support team (shelter support and housing stability staff), on site 24/7 to support residents and help them achieve their housing goals

Housing Stability Program:

• supports a caseload of individuals and families who have completed the common assessment tools
• a recovery-oriented approach to ending homelessness
• quick movement from homelessness into independent and permanent housing
• provide additional supports and services as needed

Offers health services and a variety of allied health services available to clinically rostered clients, including counselling and social work, access to registered dietitians and addictions counselling.

A program of Shalem Mental Health Network; an open arts studio where street-involved youth can begin to reconnect to themselves, others, and their community through the creative arts while gaining valuable life and work skills.

Studio is located at the Gasworks, 141 Park Street North, Hamilton.

COVID-19 Update: HIV testing is not available during the COVID-19 pandemic. Current COVID-19 precautions will remain in place until at least September 2021 for the safety of clients, staff, and the community.

Rapid Access Addiction Medicine Clinics (RAAM): Specialized medical clinic designed for people who are experiencing health issues related to their drug and/or alcohol use.

Drug Treatment Court: Specialized court process for non-violent offenders with addiction issues

Let's Grow Together: Half-day support group for women in the Guelph-Wellington community who are pregnant or parenting children (6 years and younger) and are dealing with substance use issues

Specialized Addiction Support Coordination: For specialized populations (geriatric, acquired brain injury, developmental disability, complex mental health) living with addiction issues; education and consultation to professionals working with these populations

Specialized Outreach Services: Integrated team of peers and professionals offering flexible outreach services to homeless or street-involved people with addiction, mental health, or concurrent issues

Supportive Addiction and Mental Health Housing: A range of supports for people with complex addiction and concurrent mental health needs who are homeless, at risk of becoming homeless, or who are considered inadequately housed, without secure and / or maintained housing

Supported Recovery Room: 72-hour stabilization program for people recovering from a mental health or substance related crisis. Eligibility criteria information is available through SRR (519-362-9855)

Addiction Court Support: Outreach-based support for community residents whose substance use has resulted in criminal charges.

Peer 2 Peer Overdose Response Programs: Matches Overdose Response peers with people who have overdosed by meeting them at the hospital Emergency Department to offer harm reduction and recovery support to them, their family and friends.

Rural Wellington Addictions Service: delivered in partnership with the Upper Grand Family Health Team. Provides clients with in-person assessments, treatment/care planning and counselling services.

This service works together with the Rapid Access Addiction Medicine Clinics (RAAM) and Community Withdrawal Support Service (CWSS) to provide addictions support to residents of Wellington County.

To aid vulnerable populations during the COVID-19 outbreak HAM SMaRT is distributing food, clean needles, and anti-infection supplies, as well as providing information about the virus, local resources, and recommended safety measures. They are also surveying their clients to gather data about how the COVID-19 virus is effecting these communities.

An outreach service of general internal medicine and infectious diseases physicians, registered nurses, midwifery and outreach workers whose goal is to provide excellence in clinical care to individuals who have difficulty accessing care in the traditional medical system. Acts through a mobile service specializing in patients with chronic or infections diseases who are affected by poverty, homelessness, and addiction. Operates in association with McMaster University's Department of Medicine and the Hamilton Shelter Health Network.
