Provides transportation to St Thomas vaccine site for adults age 55 and over or adults with disabilities at a cost (VON intake process must be completed, call 519-637-6408)

Free service led by ONE CARE Home & Community Support Services that provides help to older adults to settle in at home after being in the hospital. Provides transportation, meal and an attendant to help you get settled in at home. Community partners: Milverton Community Outreach, Mitchell Community Outreach, St. Marys Home Support Services, VON Perth Huron Community Support Services.

COVID19 - Service Change. ONE CARE is providing essential services. Continuing programs include Assisted Living, Meals on Wheels, Lifeline, Home Care, Home Help, Home at Last and In Home Respite. Modified programs (may be offered virtually, by phone or in a totally different way) include Transportation Friendly Visiting, Adult Day Program, Overnight Respite, Social Work, Dining (Dining to Go) and Exercise & Wellness (offered virtually). Programs on hold include Blood Pressure Clinics. In cases where a program is on hold or has had to be modified ensuring that client's needs are considered and working with clients and caregivers to determine how to support going forward.

Transportation is provided in and out of town using agency-owned vehicles (buses, accessible vans and caravans), as well as taxis and volunteers in their own vehicles.

Client must pre-register for this service and there is a set fee is applicable

Door-to-door accessible transportation service in ramp-equipped vehicles for seniors and persons with disabilities as well as those individuals without a vehicle, no family or friend to provide transportation. One attendant per client rides free

Links volunteers with those who need a ride to medical appointments, to run errands, visit or shop. 24 hours notice is required and service is dependent on volunteer availability.

Schedules, coordinates, and provides accessible transportation throughout Perth East & area in our accessible vehicles and with volunteer drivers to: medical appointments locally and out of area adult day program in Listowel social, errands Charter/group trips can be arranged through our office. After hours charters can be arranged. Masks must be worn on vehicles.

Licensed 80 bed long-term care home including resident care wings, activity areas and dining rooms Knollcrest offers: 24 hour Nursing, meals, Medical Director, on site Physio, Dietician services, Mobility Bus Service, Activation and Restorative Care programs, on site greenhouse, non-denominational Church Service on site, Music Therapy available, on site Lab service. Community Outreach Services provides services in the community and rural areas: for home support services call 519-595-8755.

COVID-19 - Essential services are running for transportation. Safety protocol is in place. Social transportation is provided based on availability. Volunteer driving is suspended until further notice. Subsidy is available for those who qualify.
