The critical care program at Grand River Hospital provides care to seriously ill patients who require intensive monitoring and specialized life saving interventions. We have two intensive care units: ICU A on the second floor (F wing), and ICU B on the fifth floor (B wing).

The medical imaging department offers a wide range of therapeutic and diagnostic exams to service outpatients, inpatients, and emergency patients.

Except as noted, all of these services are provided at the KW Campus at 835 King Street West in Kitchener.

The medicine program consists of a range of inpatient services including a general medicine unit, clinical teaching unit, acute care for the elderly unit and hospital elder life program. Links to community care resources and outpatient services are also available, including the adult diabetes education centre and internal medicine clinic.

Committed to delivering coordinated stroke services, providing care and education based on stroke best practices and evidence through collaboration with health care partners. Services include hyperacute and acute stroke care, inpatient and outpatient rehabilitation services, secondary stroke prevention clinic and neurology consultation.

The diabetes education centre provides education and support to adults with diabetes and their families.

The rehabilitation program helps people recovering from a recent illness and/or living with a chronic illness that impacts their independence with their activities of daily living

The Women's and Children's Health Program cares for families and children in a family-friendly environment, with an emphasis is on health promotion and illness prevention.

This program provides focused, short term outpatient support to individuals with diabetes to help them self-manage their diabetes care by providing them with the appropriate knowledge, skills and tools based on their needs.

Services:

• Paediatrics: Type 1 Diabetes

This service is offered as an outpatient clinic and to inpatients at Cambridge Memorial Hospital. Paediatric clinics are held monthly and supported by a paediatric team comprising a Paediatrician, Registered Nurse, Registered Dietitian, Social Worker, and a Child Life Worker.

• Adults: Type 1, Type 2

This service is offered to adults who have Type 1 and Type 2 diabetes and also includes people who get diabetes due to steroids. The DEP team also supports inpatients of Cambridge Memorial Hospital

• Diabetes and Pregnancy Clinic

The Diabetes and Pregnancy clinic is held weekly by Dr. Liutkus and a Certified Diabetes Educator.

A sleep disorders clinic serving patients in the Cambridge-Waterloo area offering a full range of services.

Community hospital serving Cambridge and North Dumfries township

Visitor restrictions are in place at Groves Memorial Community Hospital to minimize the risk of COVID-19 transmission. All visitors must be free of COVID-19 symptoms; have not have travelled outside of Canada in the previous 14 days or had contact with a sick person who has travelled; have not had close contact with a person who has/is suspected to have COVID-19; must wear a hospital-provided procedure mask during the visit; enter/exit through designated doors; adhere to infection control practices and wear the sticker identifying their successful screening process.


Patients attending the Emergency Department are limited to ONE support person.

Obstetrical patients are limited to ONE partner in care during their stay.

Stratford Site acts as a regional referral centre and District Stroke Centre for Huron Perth, along with offering a broad range of medical/surgical inpatient services including critical care and neonatal intensive care, supported by internists, pediatricians and surgical specialists such as urology, general and orthopedics. Stratford Site also provides mental health, maternal child, complete continuing care and rehabilitation inpatient services.

The Listowel Memorial Hospital is a major provider of health services for North Perth. 18 med surge beds,15 CCC beds, 5 obstetrics beds, CT scanner and specialty clinics (obstetrics, gynecology, internal medicine, surgical internal medicine, pediatrics, dermatology, orthopedics, gastroenterology and endocrinology). Training site for medical students interested in rural medicine (ROMP). Ontario Breast Screening Program site.

Outpatient Mental Health Social Worker, Katie Fergus, BSW, RSW provides brief intervention to clients needing assistance in addressing social determinates to health, including income, housing, mental health and addictions. Assists with case management, linking clients with appropriate services, completing applications for programs and funding, and provides brief counselling support until community programs are initiated. Short Term Complex Medical Management (SCMM) - Services to increase activity tolerance and progress functioning overall

Long Term Complex Medical Management (LCMM) - Services offered over an extended period of time to maintain and/or slow the rate of patient's functional losses
