The 519 Community Collective has 12 programs including a community fridge nourish kitchen, 2 urban gardens, 23 little free food pantries installed across the region, an assortment of emergency food programs, and more.

Street Outreach Program- support individuals who live, work or socialize on the street. Help to navigate systems, providing referrals to resources, and offering practical support that assists people in meeting their basic needs.

Various complex capable addiction treatment programs and services offered in the community utilizing a harm reduction philosophy. Community-based program and service options include:

Community Withdrawal Support Service: set of services offering an alternative to residential withdrawal management

Rapid Access Addiction Medicine Clinics (RAAM): Specialized medical clinic designed for people who are experiencing health issues related to their drug and/or alcohol use.

Drug Treatment Court: Specialized court process for non-violent offenders with addiction issues

Let's Grow Together: Half-day support group for women in the Guelph-Wellington community who are pregnant or parenting children (6 years and younger) and are dealing with substance use issues

Specialized Addiction Support Coordination: For specialized populations (geriatric, acquired brain injury, developmental disability, complex mental health) living with addiction issues; education and consultation to professionals working with these populations

Specialized Outreach Services: Integrated team of peers and professionals offering flexible outreach services to homeless or street-involved people with addiction, mental health, or concurrent issues

Supportive Addiction and Mental Health Housing: A range of supports for people with complex addiction and concurrent mental health needs who are homeless, at risk of becoming homeless, or who are considered inadequately housed, without secure and / or maintained housing

Supported Recovery Room: 72-hour stabilization program for people recovering from a mental health or substance related crisis. Eligibility criteria information is available through SRR (519-362-9855)

Addiction Court Support: Outreach-based support for community residents whose substance use has resulted in criminal charges.

Peer 2 Peer Overdose Response Programs: Matches Overdose Response peers with people who have overdosed by meeting them at the hospital Emergency Department to offer harm reduction and recovery support to them, their family and friends.

Rural Wellington Addictions Service: delivered in partnership with the Upper Grand Family Health Team. Provides clients with in-person assessments, treatment/care planning and counselling services.

This service works together with the Rapid Access Addiction Medicine Clinics (RAAM) and Community Withdrawal Support Service (CWSS) to provide addictions support to residents of Wellington County.

COVID-19 Update: HIV testing is not available during the COVID-19 pandemic. Current COVID-19 precautions will remain in place until at least September 2021 for the safety of clients, staff, and the community.

Support Services: counselling for persons living with HIV/AIDS to assess needs and establish a continuum of support service.

Stepping Stone helps individuals and families experiencing homelessness step forward on a path toward securing permanent housing in Guelph-Wellington through housing focused programs and individualized support for adults ad families.Areas of focus and service include:

• preventative action to help those at risk of homelessness
• individualized plans for those struggling to attain a long-term residence

Prevention & Diversion:

• prevention assistance supports households in preserving their current housing situation
• diversion support assists adults and families in finding housing outside of shelter
• prevention and diversion services help stabilize current situations while assisting with a permanent housing solution

Housing Focused Outreach:Housing Focused Outreach worker visits locations known to have individuals who are precariously housed, at risk of homelessness, or experiencing homelessness. Services include:

• assess needs of individuals and families through a standardized assessment tool
• assist with aquiring suitable housing
• identify intervention recommendations
• supports their transition to a case manager

23 Gordon St. Overnight Emergency Shelter:

• previously known as the Warming Centre
• provides coffee, shower, and overnight shelter
• accommodates up to 27 people on a first-come, first-served basis
• supported by Diversion team, Housing Stability Outreach and Housing Focused Outreach

Grace Gardens Permanent Supportive Housing:

• individuals experiencing chronic homelessness, mental health issues and / or addiction
• Mon-Sun, 24/7 support services

Housing Focused 24/7 Supportive Shelter:

• supports approximately 40 people who are experiencing homelessness
• Stepping Stone works in collaboration with community organizations to deliver mental health, addiction, and medical support to individuals staying at the shelter
• focus is to provide access to food, showers, nursing, harm reduction, and court support
• individuals have their own rooms, common space, etc.
• support team (shelter support and housing stability staff), on site 24/7 to support residents and help them achieve their housing goals

Housing Stability Program:

• supports a caseload of individuals and families who have completed the common assessment tools
• a recovery-oriented approach to ending homelessness
• quick movement from homelessness into independent and permanent housing
• provide additional supports and services as needed

Offers health services and a variety of allied health services available to clinically rostered clients, including counselling and social work, access to registered dietitians and addictions counselling.

Also offers EarlyON Child and Family Centre Programs, various programs open to community members in their priority population, such as: Parent Outreach Programs, Specialized Outreach Services, and a wide variety of healthy living programs:

WE Breastfeed: peer-led support for breastfeeding parents

Chronic Pain Programs: peer-led support groups, and self-management programs for people living with chronic pain

Mindfulness: meet each day more mindfully with greater intention and awareness

Post Partum Mood Disorder Support Group: personalized support and a weekly mother's support group with child care

ID Clinic: assistance with applications for new and replacement Canadian birth certificates, or Ontario Health Card replacement

Groceries from the SEED: nonprofit, online sliding scale grocery store, open to all, visit The Seed Guelph

Garden Fresh Box: nonprofit, fresh produce buying service to help provide affordable access to fresh fruit and vegetables. See Garden Fresh Box

Fitness Programs: various yoga, Zumba and wellness classes offered online and throughout the community

English Conversation Circle: informal groups offering newcomers a chance to practice and gain confidence in speaking English

Organization promotes community-based services responsible to the needs of those with mental health concerns, their family members, and the general community

• Alternative Activity Centre
• Brant Safe Beds Program - for individuals who are 16 years of age and older, with a mental disorder (no diagnosis required), who are in crisis that has police involvement, are able to be safely supported in the community, are not at risk of harm to selves or others and are not suitable for hospitalization and criminal detention
• Community Support Case Management Program - provides intensive case management supports to people with serious and persistent mental illness
• Specialty Case Management - including concurrent disorders (mental health and addictions), complex needs (dual diagnosis / acquired brain injury), urgent needs (needing immediate, non-crisis supports), and Transition (referrals/transition from hospital)

Outreach team consisting of an Outreach Coordinator, Nurse Practitioner, Outreach Worker, and Peer Support Worker who will connect with vulnerable people in the downtown core who may be experiencing addictions, mental health or housing challenges

Mobile team that travels throughout Brantford focusing on specific neighborhoods. This team focuses on engagement with unconnected youth 12-21 years of age and their families, with a priority on Community Development and connection to existing resources within the community

Provides emergency assistance to community members in need in Brantford and Brant County in the form of clothing, household items, furniture, nonperishable food, personal hygiene items or grocery gift cards

Provides health services for people with chronic or infectious diseases who are unable to visit the doctor's office. Services are provided by physicians, registered nurses, midwives, and outreach workers.

Community-based early intervention program that is designed to help young people who are at risk of or in the early stages of homelessness. Services offered include: family mediation and reunification, school re-engagements and retention support, intentional case management, engagement in employment, and assistance identifying and connecting to both natural and formal support networks.

Multi-agency resource centre for at-risk youth residing within the community. Case coordinator assists the youth in navigating community and internal services based on their identified needs.
Services include:

The Notre Dame Community Resource Centre, is a multi-agency resource centre for at-risk youth residing within the community who are between 16 and 21 years of age. Youth have access to a number of outreach support services and programs to assist them in remaining housed; diverting them from homelessness and street involvement.

A case coordinator assists the youth in navigating community and internal services based on their identified needs.

Street Outreach Services that engage individuals who are experiencing mental illnesses and are homeless or living in shelters. City of Hamilton Mental Health and Street Outreach staff work in the community at many locations. This team helps persons who are homeless connect to income sources, health care, housing and support services. The team works on the street, in shelters and in the community.

Drop-In program that provides spiritual and social support and connections to community services.

A street outreach program for women in the sex trade. Offers support, friendship, assitance, and prayer.

A program of Shalem Mental Health Network; an open arts studio where street-involved youth can begin to reconnect to themselves, others, and their community through the creative arts while gaining valuable life and work skills.

Studio is located at the Gasworks, 141 Park Street North, Hamilton.
