Program assists people with disabilities in finding sustainable, competitive employment or to helping to realize self-employment. A person does not need to be in receipt of ODSP in order to be eligible for Employment Supports.

Intensive Case Management Services engage individuals with a serious mental health concern who currently live in a home with a fixed address. City of Hamilton Public Health nurses work with residents to increase independence and participation in their community, as well as address health concerns. This team visits people in their home and in the community.

A comprehensive community-based treatment and psycho-social rehabilitation program dedicated to helping people with schizophrenia, through intensive case management services, social recreation, rehabilitation programs, and psychiatric services.

Provides alternatives and options for individuals experiencing serious mental illness who become involved in the criminal justice system.
This program is comprised of three components:

• Mental Health Court Diversion
  • Education regarding consumer issues, mental illness, mental health services, and the mental health system
  • Connects persons considered appropriate for diversion to access services and supports
  • Provides consultation to the judiciary on cases referred for diversion
  • Provides consultation for persons not deemed suitable for diversion
  • Support and information for family members and significant others

  • Helps support individuals with a serious mental illness, a concurrent disorder, or a dual diagnosis

Residential care facility for individuals with broad spectrum disabilities who are unable to live independently.

Services include:

A community-based program offering counselling and case management services to individuals with serious mental illness or concurrent disorders. Social work staff assess mental status, needs, and abilities to assist individuals in developing a plan and setting realistic goals. Skills training, referrals to community resources, and advocacy are provided as required.

Provides long-term intensive case management to individuals diagnosed with a combined or co-occurring substance use disorder and/or gambling disorder and serious mental illness.

Goals of the program are to:

• Reduce the likelihood of re-hospitalization
• Use appropriate community resources for client's maximum benefit
• Develop greater independence in areas of self-care, life skills, community and professional involvement

Provides rehabilitation services and skills development to adults with a serious mental illness who live in residential care facilities.

Rehabilitation Counsellors assist residents with developing various skills required for living, working and learning successfully in the community.

Individual short-term support and advocacy services available.

Instruction and support is provided to facilitate skill development in areas such as:

• cooking
• cleaning
• laundry
• budgeting
• communication
• social skills
• community involvement
• problem solving

The single point of access for all programs funded by the Ministry of Children and Youth Services and the Ministry of Community and Social Services. Helps children and youth in Hamilton that have social, emotional, behavioural, psychiatric, and/or developmental concerns find resources, services in the community. Each client is paired with a resource coordinator who will discuss support options, make referrals to other services and follow up to evaluate care plan.

A consumer-run drop-in and resource centre for people with lived experience of mental health or addiction problems. Peer support is available as needed in person and over the phone. Also provides peer support training to individuals and agencies.

The Forest Program serves youth ages 10 to 18 with a dual diagnosis and is part of the service array available through the Guiding Partnerships and Solutions (GPS) Program. A health and lifestyle program aimed at preventing chronic disease and enhancing the quality of life for those living with mental illness and offers a variety of modules (Exercise, Cooking, Nutrition, Smoking Management, and Weight Management).

Offers programs that assist people with serious mental illnesses, living both in and out of hospital, to gain or regain valued roles through evidence-based rehabilitation. A program of the Schizophrenia & Community Integration Service (SCIS).
Programs include:

• Therapeutic Recreation: Utilizes functional intervention, leisure education, and recreation participation processes to provide rehabilitation services. Service is focused on the acquisition and maintenance of resources, skills, knowledge, and behaviours which allow for independent functioning and optimal benefits of a balanced leisure lifestyle. Provides services within physical, social, cognitive, spiritual and effective domains, on-site and in the community.

Provides a central referral point to all programs, clinics, and services offered by the Developmental Pediatrics and Rehabilitation Program. The community may call to inquire about programs and services. The program provides a variety of clinics and services for children and youth with developmental, emotional/behavioural, communication, and/or physical disabilities requiring specialized services. Also provides services for adults with augmentative communication needs through our Technology Access Clinic. Part of the Hamilton Health Sciences Network.

Provides coordination and case management of community resources, advocacy, emotional support, and guidance to adult individuals with a developmental disability.
The Adult Protective Services Worker program (APSW) provides support to those living in the community on their own with little or not significant support.
The Adult Family Support Worker program (AFSW) provides support to those living at home with their family.
Services include:

• identification of a person's needs, strengths, resources, and goals
• Identification of/and liaison with additional required services
• Individualized planning

Due to COVID-19, all groups and individual appointments have been moved online.

Provides support and guidance to anyone struggling with body image issues, disordered eating and eating disorders. Services include: support groups, educational workshops, gentle yoga, support for families and friends, meditation and individual psychotherapy. Also provides an e-classroom where individuals enrolled in the treatment programs or workshops can access, download, and print materials.
