Hospital-based programs that provide multidisciplinary team assessment, diagnostic, therapeutic and rehabilitative services on an out-patient basis. Physician referral is required.

Physicians and Nurses who specialize in the assessment and treatment of elderly persons staff the clinics. Physician referral is required.
Geriatric Assessment In-Patient Units
Hospital in-patient service where patients may be admitted for in-depth multidisciplinary geriatric assessment and treatment. Physician referral is required.
Geriatric Rehabilitation Units
Hospital in-patient service where elderly persons may be admitted for individualized rehabilitation programs under the direction of a multidisciplinary team. Physician referral is required.
