• Assist community members with the transition to long-term care homes when living at home is no longer possible
• Can provide information about, and can link individuals to, other helpful services available in the community
• Care Coordinators are health care professional specially trained to help patients investigate and organize their health care services so that patients are able to achieve the best possible state of health and well being.
• Offer a single point of access to home and community services
• Services include: specialized nursing programs, nursing clinics, personal support services, occupational therapy, physiotherapy, social work, speech-language pathology, information and referral to community health and social services, services for children and youth, palliative care consultation, telehomecare, long-term care placement and more.

Community Nursing Clinics - Nursing services for ambulatory patients at clinics located in the community

Health Care Connect - Refers people who do not have a doctor to a family health care provider who may be accepting new patients * once registered, program participants are assigned to a Care Connector employed by the Home and Community Care Support Services * Care Connectors work with family doctors and nurse practitioners who are accepting new patients

In-Home Services - Health care services to people in their own homes

Information and Referral - Provides information on health care services available in the community or in-home * assesses needs of new clients * Connects clients with health care resources

Mental Health and Addictions School Program - Assessments, interventions and support to students and families with mental health and or addiction issues * consultation to school staff in regards to mental health and/or addiction issues as appropriate

Ontario Telehomecare Program - 6 month telephone coaching, support and monitoring program patients with mild to moderate Chronic Obstructive Pulmonary Disease (COPD) and Congestive Heart Failure (CHF) * equipment provided (blood pressure, weigh scale, pulse oximeter and tablet) to report on health daily * weekly discussions with telehomecare nurse

School Health Support Services - Special health services for children who require medical or rehabilitative assistance to attend school, participate in school routines and receive instruction * services are provided within public, private or home-based schools

Key features include completion of a supportive community screening; ensuring required services and community supports are in place prior to the individuals admission into long-term care; and finally, a review of life and medical history, previous agency involvement, and any access needed to continue those services for the individual once they have entered their new long-term care residence.

Provides eligibility assessment, coordination of care, placement services and system navigation for those in need of community-based health service. Assessment for in-home services and placement also provided.
In-home services include:

Placement services can include placements into Long-Term Care Homes for both long-term or short-term and adult day programs, rehabilitation and cancer continuing care beds. Access to assisted living and supportive housing

Provides access to community care, links people to a wide range of community services and supports, care coordinators work with patients/clients in community and hospital settings to determine the right care and support they require, care is delivered to the client in various community settings including home, school, residential facility and at flex clinics
