A Christian ministry supported by 30 local churches in Chatham-Kent whose volunteers have been screened and are ready to assist in the community:

• free transportation to the needy (2 rides per month)
• friendly visits
• minor home repairs
• pick up and delivery of grocery items
• advocacy where appropriate

NeighbourLink accepts referrals from the C-K Health Alliance, the local Canadian Mental Health Association office, the Local Health Integration Network and C-K Children's Services. NeighbourLink connects people in need with people who can help.

A volunteer run program that provides snow removal services for seniors, disabled and vulnerable citizens in the City of Chatham.

• The expectation is that snow will be cleared within 24 hours of any snowfall exceeding 5 cm. ( or 2 in)
• Volunteers are not required to assist for snowfalls of lesser amounts
• Volunteers are matched with one client (or more if the volunteer is willing)
• Every effort is made to match volunteers and clients who live in close proximity

Designed to help people transition back into their own home after a hospital admission, a visit to the emergency department, or a day surgery at hospital. This program can provide: a ride home, two days worth of nutritious meals, light housekeeping, laundry, grocery service, companionship or caregiver relief and referrals to other community services. Client's can receive up to 3-4 visits over the course of 3-4 weeks.

A comprehensive network of care services available to clients in urban, rural and remote communities across Ontario. A team of nurses, therapists and community support workers deliver a broad range of services ranging from chronic illness management, nutrition, physiotherapy, to personal grooming and support, all aimed at helping to ensure clients enjoy the freedom and independence that comes with staying in their own homes. Provide any level of care required, from 24 hours/ day to once a month. Services include:

Nutrition & Diet Help

• Dietitians can provide nutrition assessment and counselling for a variety of medical conditions including diabetes management and weight management
• Offer nutrition education and support around the implications associated with cancer therapy.
• Can monitor feeding and growth of infants and children and offer support for clients on enteral feeding and total parenteral nutrition.

Currently have 3 supportive housing programs, with a total of 18 fully accessible and 11 partially accessible apartments with round-the-clock non-medical care available to assist tenants with all Activities of Daily Living including but not limited to, personal care routines, hygiene, feeding, transfers, medication administration and meal preparation.

Helps older adults and persons with disabilities maintain their independence and quality of life.

Home Help

• a broker service, which matches clients to skilled, self-employed Home Helpers, based on client-identified needs and goals.
• free in-home assessment ensures all client needs are addressed.all assessments are completed by the Home Help Service Coordinator who is a registered healthcare professional.
• affordable rates range from $20-$25 per hour for a variety of services from general housekeeping to personal support and respite.

Frozen Meals

• nutritionally balanced, home-style meals developed by dietitians.
• lactose free, gluten free, minced, and pureed meals are available by request.
• pricing ranges from $2.75 to $7 for single items; savings bundles available.
• free delivery in Chatham-Kent twice monthly.

Client Intervention and Assistance

• provides short-term social work services that support and empower individuals to live independently, with dignity, in the community.
• possible services include 1:1 supports, personal assessments, service coordination, system navigation, and advocacy.
• client directed, confidential, and delivered at no cost.

CareLink Health Transit

• provides accessible, non-urgent medical transportation services.
• trips range from local to out-of-town and may include medical or therapeutic appointments, social activities and other destinations that support the activities of daily living.
• flat rate fares vary based on single destination and 3 hour service window applies.
• additional destinations can be added at a fee of $5 per destination.
• additional fees will be applied to driver wait times in excess of three hours at $20/hour.

Nurses work together with clients, caregivers and Health care professionals to assess the individual's health care needs and collaboratively develop a personalized plan of care to meet their needs. Every visit will consist of ongoing assessment, support, encouragement and holistic care.

Provides access to in-home health and personal support services to help individuals live independently at home

• Assist community members with the transition to long-term care homes when living at home is no longer possible
• Can provide information about, and can link individuals to, other helpful services available in the community
• Care Coordinators are health care professional specially trained to help patients investigate and organize their health care services so that patients are able to achieve the best possible state of health and well being.

Dementia Care Services

Provision of on-site Direct Support Professionals in housing situations up to 24 hours per day, 7 days per week at various locations in Chatham-Kent. Supports are based on people's abilities and wishes.

Provides and advocates for a full range of community-based services for people who have intellectual disabilities including accommodation, employment, respite, community access, planning and family support.

COVID-19 - Offices are closed to the public, however, all support activities are being conducted.

• In home services - include personal care and home management (light housekeeping, shopping and meal preparation).
• Seniors Program - a variety of programs are offered throughout the year such as drop-ins, craft workshops, shopping and game nights.
• Seniors Congregate Dining - takes place Wed at Community Centre. Free lunch served to community members ages 55 and up.
• Medical Loan Cupboard - A limited amount of medical equipment is available for lending. Availability depends upon community need.
