AGE WELL
Available to community members who are in their senior years, are at a risk for falls, and/or living with a chronic health condition. The Age Well team consists of a Registered Nurse, Occupational Therapist and a Physiotherapist. In partnership with the client, the team provides practical tools, knowledge, and skills clients need for safe and independent mobility in their homes and community and to optimize their management of chronic health conditions. Some of the programs and services included are home safety assessments, exercise programs, support groups and a grab bar program. For information, call 519-397-5455 ext 225.

A healthy lifestyle program designed to help you feel better, be healthier and reach your weight loss goals. This program focuses on small changes that have a big impact! This program assists in identifying and overcoming risk factors and barriers, creating healthy eating habits, and designing a healthy physical activity routine. Led by a Respiratory Educator and Physiotherapist to help those living with Chronic Obstructive Pulmonary Disease (COPD) build self-management skills and exercise tolerance. Participants learn about medication management, action planning, breathing techniques and more. This program is in partnership with the Home and Community Care Support Services Erie St Clair. CARDIAC REHAB AND SECONDARY PREVENTION
Outpatient program for men and women with known cardiac disease such as a previous heart attack or heart surgery. Participants are offered a 6-month supervised exercise program with general risk factor education and counselling as needed in nutrition, stress management, medications and general information on heart attack and stroke. 6-week workshop focuses on enabling clients to self-manage their chronic diseases. With an emphasis on goal setting, action plan development, and problem solving, group members will work together and support each other in a journey towards empowerment over their own health. Each program session includes education on different aspects of chronic disease conditions. This program is offered in both English and French. For information, call 519-397-5455 ext 117.

Provides supportive care to children and teens aged 4-18 who are facing a life-threatening illness themselves, the life threatening illness of a loved one, or who have experienced the death of a loved one.
Support is offered through age-appropriate activities and education that help the child grieve in a safe and healthy manner while giving them the tools needed for normal, healthy grieving. The program is centered on the unique needs of each child and the focus is to provide one-to-one and group support for children and teens in an environment where they can feel safe to share their feelings.
Visits take place at the child's school, in hospitals, homes or the child-friendly space at the VON office. There is no limit to the number of support sessions.

Augmentative Communication Services (ACS) General Level Clinic helps children, youth and their families find ways to communicate. The Ministry of Health Assistive Devices Program authorizes Children's Treatment Centre to provide appropriate ACS services to children and youth with home based complex communication needs. Eligibility is restricted to children aged 19 or younger, but clients of the program may be monitored for ongoing communication needs and remain on the case load if needed. Speech Language Pathologist, Occupational Therapists and Communication Disorders Assistants all with additional training in AAC (Augmentative and Alternative Communication). Together the team supports children, youth and their families who have communication challenges through the prescription and customization of AAC devices, training and consultation in all environments over time. Augmentative and Alternative Communication (AAC) Clinic Form and Principal Referral Form are to be completed with parent's consent by the school principle and submitted by FAX, mail or email the forms to mball@CTC-CK.com.

- Hearing aid prescription/evaluation- to discuss the use of hearing aids, provide information and prescribe a hearing aid
- Hearing aid dispensing/selling hearing aids
- Hearing aid re-check
- Central Auditory Processing testing- for children between the ages of seven to thirteen, who may be experiencing learning difficulties
- Otoacoustic Emissions testing- to test the reliability and function of the cochlea (organ of hearing)
- Identifies infants who are born deaf or hard of hearing or who are at risk of hearing loss in early childhood
- Provides parents of deaf or hard of hearing infants with the services needed to develop communication and language skills
