Searching for Transportation To Medical Appointments

A Christian ministry supported by 30 local churches in Chatham-Kent whose volunteers have been screened and are ready to assist in the community:

• free transportation to the needy (2 rides per month)
• friendly visits
• minor home repairs
• pick up and delivery of grocery items
• advocacy where appropriate

NeighbourLink accepts referrals from the C-K Health Alliance, the local Canadian Mental Health Association office, the Local Health Integration Network and C-K Children's Services. NeighbourLink connects people in need with people who can help.

Transportation service that focuses on connecting people to their community by providing an affordable, accessible transportation option to rural and/or low-income people. The program has one accessible vehicle that can hold 2 passengers using wheelchairs and 7 walk on passengers.

Eligible residents can use the transportation service for a variety of needs ranging from drives to appointments and social activities or to be linked to pre-existing transportation services within Chatham-Kent such as the bus or VIA Rail.

Drives will be provided to multiple passengers at the same time, based on destination. Transportation service is offered on a first come, first serve basis.

Passengers must provide the Transportation Office with at least 3 business days (72 hours) notice to book a drive, whenever possible. Short-notice requests may not be fulfilled, based on passenger capacity.

Helps older adults and persons with disabilities maintain their independence and quality of life.

Home Help

• a broker service, which matches clients to skilled, self-employed Home Helpers, based on client-identified needs and goals.
• free in-home assessment ensures all client needs are addressed.all assessments are completed by the Home Help Service Coordinator who is a registered healthcare professional.
• affordable rates range from $20-$25 per hour for a variety of services from general housekeeping to personal support and respite.

Frozen Meals

• nutritionally balanced, home-style meals developed by dietitians.
• lactose free, gluten free, minced, and pureed meals are available by request.
• pricing ranges from $2.75 to $7 for single items; savings bundles available.
• free delivery in Chatham-Kent twice monthly.

Client Intervention and Assistance

• provides short-term social work services that support and empower individuals to live independently, with dignity, in the community.
• possible services include 1:1 supports, personal assessments, service coordination, system navigation, and advocacy.
• client directed, confidential, and delivered at no cost.

CareLink Health Transit

• provides accessible, non-urgent medical transportation services.
• trips range from local to out-of-town and may include medical or therapeutic appointments, social activities and other destinations that support the activities of daily living.
• flat rate fares vary based on single destination and 3 hour service window applies.
• additional destinations can be added at a fee of $5 per destination.
• additional fees will be applied to driver wait times in excess of three hours at $20/hour.

Transports clients requiring non-urgent stretcher transportation to and from appointments in Windsor, London or Toronto.

• attendants will wait for you while you are in your appointment and then bring you back home.

Health centre focused on family and community health, illness prevention and health promotion * health professionals assess and treat non-life-threatening injuries or illnesses * health professionals provide primary care services and referrals to other local health and community services and personal development groups

Community-led health teams develop programs to improve the social supports and conditions that affect long-term health

Transportation to Vaccine Clinics - Volunteer drivers take West Elgin residents to vaccine clinics * accessible shuttle for those who need it * funded through Gift-A-Ride program * working to get everyone who needs a vaccine access to a vaccine

Assisted Living Program - Support staff provide care for frail older adults or people with disabilities, who have difficulty caring for themselves, but who are still capable of living independently in their own home or apartment * personal support, homemaking, care coordination, medication assistance, and security checks * scheduled and unscheduled visits * support staff available daily in the homes/apartments of clients

Community Support Services for Seniors - Various programs provide supports to help older adults maintain independence while remaining in their home and community

Health Promotion and System Navigation Services - One-on-one and group sessions to promote health and to assist people in navigating the health system connecting with community programs and linking to available social services

Parenting Support - Parenting education and support including prenatal classes, breastfeeding support, and a Well Baby Clinic

The main focus of these services is to help with medical transportation needs and to help community members navigate Health Canada's Non-Insured Health Benefits System.

The Non-Insured Health Benefits Program is Health Canada's national, medically necessary health benefit program that provides coverage for benefit claims for a specified range of drugs, dental care, vision care, medical supplies and equipment, short-term crisis intervention-mental health counseling and medical transportation for eligible First Nations people and Inuit.



The NIHB Program will provide coverage for a limited range of medically necessary goods and services to registered members. It may repay some or all of the costs incurred when health care programs, products or services are purchased. This includes a range of medical, dental and pharmacy services when they are not covered by provincial or other third party health plans.

Medical Transportation - Services all members of Aamjiwnaang living on-reserve who travel to any location within catchment area * Note that transportation to Dialysis, Cancer Treatments and/or Methadone Maintenance Programs will need to be approved ahead of time. Speak with Health Services Clerk to arrange transportation assitance.

COVID-19 - Essential services continue with pre-screening protocols for Transportation, Meals on Wheels, Personal Support and Crisis Intervention. Non-urgent stretcher transportation is designed specifically for stable, non-emergency patients who reside in Lambton County, requiring transportation from home to hospital, hospitals to hospital or hospital to home. The home must be equipped with a ramp and/or main floor direct access.

Provides transportation for the frail, elderly and disabled in the community. The service is for those unable to use public transportation or private means. In town; trips to medical appointments, day programs, groceries and social outings. Out of town transportation to medical appoints only with destinations along the London-Windsor corridor.
