You can establish your identity for replacement of documents by swearing an affadavit before
- a notary public
- free public Commissioner of Oaths service at Centre for Skills Development and Training, for more information see here

It may be useful to bring anything which shows your name, address etc (such as a piece of mail or utility bill)

Bring a personal identification document (verifying your name, date of birth and signature) to a ServiceOntario location

* if someone loses all of their identification or even some of their critical identification they can swear an affidavit in the presence of the Commissioner of Oaths, at the Clerk's Department at the Town Hall
* this affidavit can then be used to assist them in replacing that identification
* the individual will be required to write out their personal information such as full name, birth date, permanent place of residence, what was lost or stolen, and the particulars of how the identification came to be lost or stolen
* they must produce current and valid photo identification bearing their signature
