Provides case management support to youth ages 16-24 who are homeless or at risk of homelessness in:

Host Homes initiative:
* matches youth with host homeowners who provide safe temporary housing

Bridge House program:
* provides transitional housing for youth for a period of up to 2 years in residential neighbourhoods in Burlington and Milton

Oakville Residence
* provides transitional housing for a period of up to 2 years in a bachelor style apartment

Service Navigator (north-end Halton only)
* connect youth ages 14-24 with community supports and case management

Provides the basic needs of life: shelter, food, clothes, safety and support

Services include:

• accommodation
• pregnancy counselling
• support services during pregnancy, childbirth and for a period after the birth for the young women, their babies and family members

Program introduces a temporary housing option for eligible newcomers in the community

• Burlington based apartments and townhouses for families and individuals who are homeless or at risk of homelessness
• Provides temporary accommodation while residents search for permanent housing in Halton
• Placement is up to 12 weeks with a possibility of extension
• Rental Agreement is signed every three months with possibility of extension to up to one year

Provides furnished transitional housing apartments and support to young mothers with on-site programming and support staff. Offers supportive programs to those in the transitional housing.

Provides a place of sanctuary, safety and support for women (and their children) with a variety of programs.

Family Court Support Worker Program

• Provides information on the family court process, assistance in preparing for family court proceedings (documenting history of abuse, accessing Legal Aid), referral to specialized services and supports in the community, safety planning, and court accompaniment to proceedings, when appropriate.

• Help women and their children access services to establish violence and abuse-free lives in the community.

Second Stage Services

• Offer furnished, affordable housing to both single women and women with children for up to one year. Supports are geared to creating greater physical, emotional and financial stability in the aftermath of violence and abuse.

Provides clean, safe and affordable transitional housing for up to 174 men at risk. Short-term accommodations provided on a weekly or monthly basis with 24-hour front desk support, including navigation to critical community support services with key partner agencies.
Includes non-smoking furnished rooms, shared bathrooms, coffee shop, and free laundry facilities. Overnight security.

A 65-bed residence providing safe, affordable and temporary housing to women experiencing poverty, homelessness and violence.
Services include:

• life skills training
• individualized case management
• one-on-one support
• client advocacy

Volunteer program

Assists people to explore and fulfill life expectations beyond psychiatric stabilization and health maintenance using a partners in well-being approach. Oversees 9 independently-owned residential homes in urban and rural settings and operates day programs in 2 skills centres as a provider within the Ministry of Health and Long-Term Care residential care program for adults with serious mental illnesses. Part of the Schizophrenia & Community Integration Service.

Skills Centre programs are developed according to requests and/or needs identified by participants and may be prevocational, avocational, or related to life skills, or recreational social activity. Helps participants acquire transferable skills, achieve optimum level of functioning and well-being, and integrate into community.

Provides a residence and supports for young people who are pregnancy and new mothers. The length of stay is dependent on individual needs. Staff work closely with mothers to develop their individual goals/life plans.

• Pre-natal Program - focus is for women to experience a healthy pregnancy and birth as well as learning self care and life skills necessary to transition to their new roles as mothers
• Post-natal Program - focus is for new parents and their infants to get to know each other and for the new parents to learn parenting skills and infant care

Offers a full range of supportive housing options for those with mental health and/or substance use issues, from highly intensive staff supported residences, to community supports through rent subsidies with private landlords

• supports individuals to transition to more independent housing

• supports individuals aged 16 and up with Residential Counselors between 5-7 days per week

• has five bedrooms in a congregate setting

Grace House

• Halton's only 24/7 staffed supportive housing residence

• has 10 bedrooms in a congregate setting

Offers transitional housing and support programs for up to 12 months for Indigneous women and children who are at risk of homelessness and poverty. The supportive housing facility is geared towards women and children who have been victims of violence.

Replacing the Mountain View Program (Discontinued in September 2020), Gyo Geh Doh is a transitional supportive housing program for single women in precarious housing conditions. It has an emphasis on wrap-around supports.

Provides transitional support services in Housing, Life Coaching, Life Skills Training and Mentoring, to vulnerable and racialized youth aging out, or recently aged out of the child protection and foster care system

The Oasis House is a spacious house, in an accessible location in the North-West end of Oakville, serving mostly black male youth (18-21yrs) from the Halton/Peel Region

Volunteer program

Working in partnership with community groups, provides supportive transitional housing and program supports to assist low to moderate income families and individuals, the homeless, and those at risk of homelessness in the community.

Provides basic life skills program and counselling services to address individualized barriers to self-sufficiency through a partnering agreement with community service providers as well as 'in house' programs. Program areas include financial management and counselling, goal setting, job skills, coping strategies, etc.
