An in-home behavioural consultation service for caregivers and their children ages 1.5-17 years old
Services Include:

• In-home behavioural consultation: including assessment, goal development, and parent training/coaching
• Behavioural therapy: for children diagnosed with autism spectrum disorder and other developmental disabilities

Services delivered by a Board Certified Behaviour Analyst (BCBA)
Goals might include:

• language and communication
• self-help, pre-academics
• social, emotional regulation
• decreasing harmful behaviour such as self-injury or aggression

Students from Wilfrid Laurier University are matched with members of Community Living Brant who have intellectual disabilities

• the club hosts monthly group events
• promotes social integration
• works in conjunction with community agencies

On-line registry for vulnerable people

• registry provides quick access to critical information about a registered person, such as who to call in an emergency, a detailed physical description and any particular sensitivities that a person may experience
  
• registry promotes communication between vulnerable persons, the persons who support them and the police

This registry is a partnership between local community agencies and Brantford Police Service

Group living for adults with mental health challenges and developmental disabilities

Individualized community supports for people who live in their own home or with their family

Supports may include:

• help to connect to the community
• resourcing and networking
• advocacy/education
• person centered planning
• assistance with transition from school to community life
• developing relationships and social roles
• understanding rights and responsibilities
• assistance with landlord/tenant issues
• budgeting and money management
• home maintenance

Fee-for-service supports for those who have received Ministry of Children, Community & Social Services (MCCSS) Passport Funding

Supports that can be purchased:

• community involvement - participate in sports, recreation and leisure activities, attend club groups or local community events, getaways and day trips
• education/literacy - support to attend education or literacy programs
• employment/volunteering - job preparation, support with on-the-job training, job coaching, assistance in finding and maintaining volunteer work in the community
• person-directed planning - assistance to develop a person-directed plan that builds on strengths and interests and to identify the support needs required to reach goals
• respite - in or out-of-home respite support options

Provides and advocates for a full range of community-based services for people with developmental disabilities including accommodation, employment, respite, community access, planning and Passport supports

Program provides family-centered services in the early years of a child's development, primarily in the home. Developmental Consultants partner with families to enhance the growth and development of children and provide support, information and early intervention services

• provides continued learning for adults with developmental challenges after they leave high school

Services provided:

• special consultations are available for children on the Autism spectrum or with other Behavioural and Learning disabilities

Support group for adults with Acquired Brain Injuries or Stroke and possibly some communication challenges

• social opportunities for adults with similar health concerns
• cognitive and emotional education to enhance the quality of life
• opportunity for caregivers and clients to meet for support with the Social Worker and Speech-Language Pathologist

Short-term supports for children and youth registered with the Ontario Autism Program who are experiencing new or worsening high-risk behaviours

URS Coordinators work with the family and community services to develop short-term services and supports to respond rapidly to the specific high-risk need to reduce the risk of harm to the child/youth, others, or property

First point of contact for children and youth ages 0-18 with mental health concerns, intellectual disabilities, Autism or FASD. First place to contact for information, questions, and access to community resources and programs

Help navigate services available to children, youth, and their families. Complete an intake and make referrals for children and youth to get the services they need when dealing with:

Lead agency for Service Coordination Brant which provides coordinated service planning to families of children and youth with multiple service needs. Service Coordination ensures there is a clearly identified staff for each family to lead their service plan and support services working together to meet each family's unique needs

Lead agency for the Ontario Autism Program - Urgent Response Service - short-term supports for children registered with the Ontario Autism Program who are experiencing new or worsening high-risk behaviours

Manages the Community Information Brant, Haldimand & Norfolk database www.info-bhn.ca containing information on local community programs and services

Programs directed to children and adolescents experiencing social, emotional and behavioural difficulties. These age-specific programs include office-based individual and family counselling, group therapy, suicide prevention, day treatment, and intensive services

• Office-based counselling provides support to families whose children and youth struggle with mental health concerns, school difficulties, family conflict, divorce, trauma, grief, addictions, behavioural concerns, suicide concerns, suicidal ideation, and general counselling

• Intensive Services are directed toward families of children (0-18 years) * This program offers counselling and support, parenting strategies, consultation, and support to schools, parenting groups, and children's programs

• Tele-Mental Health provides Program Consultations, an Educational Component, and Direct Consultations with Child Psychiatrists throughout Ontario using video conferencing
