Transportation:
Provides frail seniors, and adults over the age of 18 with a physical or cognitive condition needing a non-medical escort local and long distance drives to medical appointments, central Home and Community Care Support Services funded ADP's, grocery shopping, personal errands and social outings. Priority is given to clients requiring drives to chemotherapy and dialysis appointments etc and other medical appointments before errands, shopping and outings. Clients are encouraged to call as far in advance as possible to arrange transportation.

Referrals to local services to provide maintenance services such as snow removal, lawn maintenance, fall clean up, spring clean-up, eavestrough cleaning and exterior window cleaning * program provides adaptation services within the home once a free in-home safety assessment is completed * minor handyman repairs can be provide if materials are provided

Provides homemaking services and foot care to low income York Region residents who have a temporary or permanent illness or disability so that they can remain in their homes

King -- Township of King provides services to older adults and persons with disabilities who qualify

• qualified applicants must ensure all obstructions are removed from the end of their driveway and their house number must be visible

• may take up to 24 hours from the end of storm for snow to be removed

Markham -- clears row of snow (snow windrow) created at end of driveway after street plowed for older adult residents and residents with physical disability

• crews clear centre portion of snow windrows (one car width only) on all roads following a 7.5 cm (3) snow fall
• windrows will be cleared within 8 hours after crews plow the road (City reserves the right to decide when snow windrow service will take place)
• windrow left from sidewalk plow not included in service
• those receiving this service must: remove any obstructions at the end of driveway, keep house number visible and illuminated at all times, notify the City if they move from the address on file or if at any time a person(s) moves into the home on a permanent and/or temporary basis
• residents aged 60 and older or 12 and younger (proof of age required)
• residents between 12 and 60, current and valid doctor's certificate confirming they are physically unable to clear snow must be provided
• residential homes with rear lanes do not qualify for this service

Richmond Hill -- windrow clearing service for residents

• removal make take place 13 hours after end of snowfall
• residents must keep the end of their driveway free from obstructions
• those not wishing service may opt out, must submit opt-out request before start of winter season, deadline November 15, once have opted out for season cannot opt back in

Vaughan -- provides windrow-clearing service for all residential driveways

• windrow machines clear about 80 per cent of snow left by plows at the end of the driveway within four hours after the plow has passed, not complete driveway

Whitchurch-Stouffville -- residential sidewalk and windrow snow clearing service to older adult residents and residents with physical disabilities where there is no able bodied person living in the household at no cost

• windrows may be cleared up to 12 hours after road snow clearing ends
• applicants must remove any obstructions from end of driveway and have house number visible and illuminated

Offers support to people who have a developmental disability and live on their own * The amount of support provided is dependent upon the stated needs of each person

In-home support to assist persons with routine household activities and instrumental activities of daily living, such as light housekeeping (of personal living space), household management tasks, shopping and meal preparation * assessment, service planning and care coordination is also provided * the program focus is strengths based-supporting persons to complete tasks independently and assisting with tasks they cannot perform

Service is provided by a trained Home Support Worker with special knowledge in working with older adults

COVID-19 - Staff are working remotely

Ministry funded community participation and accommodation supports for individuals with an intellectual disability who are deemed eligible for services through Developmental Services Ontario.

CLASS also offers respite accommodations or day program supports based on availability on a fee for service basis for youth and adults

Integrated Dialysis Care Program -- Registered Nurses and Registered Practical Nurses visit home peritoneal dialysis patients at their home, health maintenance, nutritional counselling

Wellness and Social Program -- social, recreational and educational programs for persons 50 years and older, drop-in centre, friendship groups, interest classes, outings

Support to individuals with developmental disabilities in Barrie and Orillia who reside in their own apartment, townhouse, rooming house or detached home setting and occasionally when still living at home
Provide support with:

• banking, budgeting, grocery shopping and meal preparation, varies among individuals and can fluctuate as situations arise
• choices of working, volunteering or pursuing leisure activities
• accessing community services as needed

If support needs decrease to a minimum, a referral to the Community Resources Worker Program would be a consideration
